新诊断2型糖尿病患者甘油三酯-葡萄糖指数及相关指标与内脏型肥胖相关性的研究

摘　　要：目的 探讨新诊断T2DM患者TG-葡萄糖(TyG)指数及相关指标与内脏型肥胖(VO)的相关性。方法 选取2022年1月至2023年12月我院标准化代谢性疾病管理中心诊治的新诊断T2DM患者833例,按TyG指数三分位数分为<8.765的Q1组(n=276)、8.765~9.266的Q2组(n=280)及≥9.266的Q3组(n=277),比较各组一般资料及FPG、FIns、HbA_(1)c、肝肾功能、血脂、胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)、Ty G指数等。Spearman相关分析TyG指数及相关指标与其他指标的相关性。Logistic回归分析VO的影响因素,绘制TyG指数及相关指标评估新诊断T2DM患者VO的受试者工作特征曲线。结果 Q1、Q2、Q3组吸烟史、饮酒史、高脂血症占比、FPG、FIns、TG、谷丙转氨酶(ALT)、HOMA-IR、BMI、WC、WHR、腰高比(WHtR)、Ty G指数相关指标(TyG-BMI、Ty G-WC、Ty G-WHtR)、内脏脂肪面积(VFA)、皮下脂肪面积依次升高(P<0.05),年龄、HDL-C依次降低(P<0.05)。Spearman相关分析结果显示,TyG指数及相关指标均与VFA呈正相关(r_(TyG)=0.318,r_(TyG-BMI)=0.661,r_(TyG-WC)=0.647,r_(TyG-WHt),r_(TyG-WHtR)=0.597,P<0.05)。Logistic回归分析显示,校正混杂因素后,Q3组发生VO的Ty G指数、Ty G-BMI、Ty G-WC、Ty G-WHtR是Q1组的2.998、7.499、7.623和6.900倍。Ty G指数、Ty G-BMI、Ty G-WC、Ty G-WHt R评估VO的曲线下面积分别为0.665、0.833、0.832、0.797,截断值分别为8.78、218.06、811.65、4.80。结论 新诊断T2DM患者Ty G指数及相关指标均与VFA呈正相关,可作为评估VO的简易指标。Objective To explore the correlation between triglyceride glucose(TyG)index and its related indicesand visceral obesity(VO)in patients with newly diagnosed type 2 diabetes mellitus(T2DM).Methods A total of 833 patients with newly diagnosed T2DM were enrolled in this study from the Standardized Metabolic Disease Management Center in our hospital during Janurary 2022 to December 2023.General clinical characteristics,physical examination indices,fasting plasma glucose(FPG),FIns,HbAjc,liver and kidney function,blood lipids were measured.Homeostasis model assessment of insulin resistance(HOMA-IR),homeostasis model assessment ofβ-cell function(HOMA-β),TyG related indices were calculated.All the patients were divided into three groups according to tertiles of TyG:Q1 group(TyG<8.765,n=276),Q2 group(TyG 8.765~9.266,n=280),and Q3 group(TyG≥9.266,n=277).The differences of general clinical characteristics,FPG,FIns,HbA_(1)c,hepatic and renal function,blood lipid,HOMA-IR,HOMA-βand TyG were compared among the three groups.Logistic regression analysis was used to estimate the association between TyG related indices and VO.The predictive value of TyG related indices for newly diagnosed T2DM patients with VO was evaluated by receiver operating characteristic(ROC)curve.Results With the increase of TyG,the smoking and drinking rates,the prevalence of hyperlipidemia,FPG,FIns,TG,alanine aminotransferase(ALT),HOMA-IR,BMI,WC,WHR,waist height ratio(WHtR),TyG related indices(TyG-BMI,TyG-WC,TyG-WHtR),VFA and subcutaneous fat area gradually increased in turn(P<0.05).Age and HDL-C decreased in turn(P<0.05).The Spearman correlation analysis showed that TyG related indices were positively correlated with VFA(r_(TyG)=0.318,r_(TyG-BMI)=0.661,r_(TyG-WC)=0.647,r_(TyG-WHt),r_(TyG-WHtR)=0.597,P<0.05).Binary Logistic regression analysis indicated that after adjusting for confounding factors,the OR values for VO occurrence of TyG,TyG-BMI,TyG-WC,and TyG-WHtR were 2.998,7.499,7.623,and 6.900 times higher in the Q3 groups than in the Q1 group,respec
