摘 要:目的探讨剪切波弹性成像(SWE)对乳腺癌不同分子分型新辅助化疗效果的预测价值。方法选取乳腺癌患者80例为研究对象,根据分子分型分为Luminal A型、Luminal B型及人表皮生长2因子受体(HER-2)过表达型和三阴性型4组,所有患者均行新辅助化疗治疗4个周期,化疗前后均接受超声剪切波弹性成像(SWE)检查,分析SWE参数Emax、Emean、SR化疗前后的变化,预测新辅助化疗的效果。结果80例乳腺癌患者化疗前Luminal A型、Luminal B型、HER-2过表达型、三阴性型4组SWE参数Emax、Emean、SR比较差异均有统计学意义(P<0.05)。化疗4个周期后,反应组SWE各参数均明显低于化疗前(t=-42.672,P<0.05)。结论不同分子分型乳腺癌患者采用新辅助化疗治疗时,化疗前后超声剪切波弹性成像的各参数指标均有不同变化,能为新辅助治疗的方案制定和调整提供可靠依据。Objective To explore the predictive value of shear wave elastography(SWE)of neoadjuvant chemotherapy for different molecular types of breast cancer.Methods 80 cases of breast cancer patients were collected.According to molecular types,they were classified into Luminal A,Luminal B,and human epidermal growth factor receptor-2(HER-2)overexpression type and triple-negative type.All patients were treated with four cycles of neoadjuvant chemotherapy,and underwent ultrasound SWE examination before and after chemotherapy.The changes of SWE parameters of Emax,Emean and SR before and after chemotherapy were analyzed to predict the effect of neoadjuvant chemotherapy.Results Before chemotherapy,the SWE parameters Emax,Emean and SR of 80 breast patients in the four groups of Luminal A,Luminal B,HER-2 and triple-negative types were all statistically different(P<0.05).After four cycles of chemotherapy,all parameters of SWE in the response group were significantly lower than those before chemotherapy(P<0.05).Conclusion The parameters of ultrasound SWE change differently when neoadjuvant chemotherapy is used for different molecular types of breast cancer patients,which can provide a reliable basis for the formulation and adjustment of neoadjuvant therapy.
