摘 要:目的:探讨移植管内黏液血染对冻融胚胎移植(frozen-thawed embryo transfer,F-ET)后临床妊娠结局的影响。方法:对2021年9月—2022年9月在南京市妇幼保健院接受F-ET治疗的患者进行回顾性分析。根据移植管内是否存在黏液或血染,将患者分为无黏液血染组(A组)、黏液组(B组)、血染组(C组)、黏液+血染组(D组),并收集患者基本信息、治疗过程及妊娠监测数据(生化妊娠率、临床妊娠率、流产率、活产率)。分析4组患者基本资料,并采用单因素分析与多元Logistic回归分析,探究黏液血染对患者临床妊娠结局的影响。结果:经Logistic回归校正混杂因素后,黏液血染是影响临床妊娠及活产的危险因素(aOR=0.852,95%CI:0.742~0.979,P=0.024;aOR=0.820,95%CI:0.713~0.943,P=0.005),并会造成流产风险升高的趋势(aOR=1.221,95%CI:0.976~1.528,P=0.081),但差异无统计学意义。结论:黏液血染对F-ET后的临床妊娠结局具有负面影响,可能导致临床妊娠率和活产率降低,流产率升高。Objective:To investigate the impact of mucous blood staining on clinical pregnancy outcomes following frozen-thawed embryo transfer(F-ET).Methods:This retrospective study included patients who underwent F-ET treatment at our institution from September 2021 to September 2022.Patients were categorized into four groups based on the presence of mucous or blood staining in the transfer catheter:no mucous blood staining group(group A),mucous group(group B),blood staining group(group C),and mucous+blood staining group(group D).Basic information,treatment processes,embryo quality assessments,and pregnancy monitoring data(biochemical pregnancy rates,clinical pregnancy rates,miscarriage rates,and live birth rates)were collected.Subsequently,we analyzed the basic data of the four groups and employed univariate analysis and multivariate Logistic regression to explore the influence of mucous blood staining on clinical pregnancy outcomes.Results:After adjusting for confounding factors using logistic regression,mucous blood staining was identified as a risk factor affecting clinical pregnancy and live birth(aOR=0.852,95%CI:0.742-0.979,P=0.024;aOR=0.820,95%CI:0.713-0.943,P=0.005),and it was associated with a trend towards increased risk of miscarriage(aOR=1.221,95%CI:0.976-1.528,P=0.081),although the difference was not statistically significant.Conclusion:Mucous blood staining negatively affects clinical pregnancy outcomes following F-ET,potentially leading to lower clinical pregnancy and live birth rates and higher miscarriage rates.
