摘  要:目的:探讨南通地区健康体检人群尿酸(uric acid,UA)水平及相关危险因素,为疾病防控提供理论依据。方法:选取2024年1—7月于南通大学附属医院体检中心进行健康体检的人群共20670例,其中男10142例,女10528例。按照性别和年龄分组,检测血清尿酸(serum uric acid,SUA)水平,统计高尿酸血症(hyperuricemia,HUA)检出率。分析HUA相关危险因素,并采用Logistic回归进行评估。结果:20670例健康体检者SUA为342(283,410)μmol/L,其中男性SUA为398(346,454)μmol/L,HUA检出率为38.67%,女性SUA为294(255,340)μmol/L,HUA检出率为17.30%。按年龄划分为7组(≤20岁、>20~30岁、>30~40岁、>40~50岁、>50~60岁、>60~70岁、>70岁),男性SUA水平和HUA检出率随年龄的增长逐渐降低,女性SUA水平和HUA检出率随年龄的增长先降低后升高。HUA组(男性SUA>420μmol/L,女性SUA>360μmol/L)中ALT、AST、谷氨酸氨基转移酶(gamma-glutamyl transferase,GGT)、乳酸脱氢酶(lactate dehydrogenase,LDH)、总胆红素(total bilirubin,TBIL)、空腹血糖(fasting blood glucose,FBG)、血尿素(blood urea nitrogen,BUN)、肌酐(creatinine,CR)、胆固醇(cholesterol,CHO)、三酰甘油(triglycerides,TG)、低密度脂蛋白胆固醇(low-density lipoprotein cholesterol,LDLC)水平显著高于正常UA组,而估算肾小球滤过率(estimated glomerular filtration rate,GFR)和高密度脂蛋白胆固醇(high-density lipoprotein cholesterol,HDLC)水平明显低于正常UA组。且HUA组中SUA与ALT、AST、GGT、TBIL、FBG、BUN、CR、CHO、TG、LDLC呈正相关,而与GFR和HDLC呈负相关。Logistic回归分析显示,男性、ALT、GGT、TBIL、BUN、TG和LDLC均为HUA的危险因素,而年龄、FBG、GFR、CHO和HDLC均为HUA的保护因素。结论:南通地区体检人群HUA检出率较高,男性、ALT、GGT、TBIL、BUN、TG和LDLC是HUA发病的危险因素。Objective:To investigate the uric acid(UA)levels and related risk factors in the healthy population undergoing physical examinations in Nantong region,and to provide a theoretical basis for disease prevention and control.Methods:A total of 20670 individuals who underwent health examinations at the Medical Examination Center of the Affiliated Hospital of Nantong University from January to July 2024 were selected,including 10142 males and 10528 females.They were grouped by gender and age,and their serum uric acid(SUA)levels were measured to calculate the detection rate of hyperuricemia(HUA).The related risk factors for HUA were analyzed and evaluated using multivariate logistic regression.Results:Among the 20670 healthy individuals,the median SUA level was 342(283,410)μmol/L.For males,the SUA was 398(346,454)μmol/L with an HUA detection rate of 38.67%,while for females,the SUA was 294(255,340)μmol/L with an HUA detection rate of 17.30%.The participants were divided into seven age groups(≤20 years,>20-30 years,>30-40 years,>40-50 years,>50-60 years,>60-70 years,>70 years).For males,both SUA levels and HUA detection rates gradually decreased with age,whereas for females,they first decreased and then increased with age.In the HUA group(for males,SUA>420μmol/L,and for females,SUA>360μmol/L),levels of ALT,AST,gamma-glutamyl transferase(GGT),lactate dehydrogenase(LDH),total bilirubin(TBIL),fasting blood glucose(FBG),blood urea nitrogen(BUN),creatinine(CR),cholesterol(CHO),triglycerides(TG),and low-density lipoprotein cholesterol(LDLC)were significantly higher than those in the normal UA group,while estimated glomerular filtration rate(GFR)and high-density lipoprotein cholesterol(HDLC)levels were significantly lower.All differences were statistically significant.Furthermore,in the HUA group,SUA was positively correlated with ALT,AST,GGT,TBIL,FBG,BUN,CR,CHO,TG,and LDLC,but negatively correlated with GFR and HDLC.Logistic regression analysis showed that male gender,ALT,GGT,TBIL,BUN,TG,and LDLC were all risk factors f
