熊胆粉、土茯苓、鬼箭羽治疗肝毒经验——仝小林三味小方撷萃

摘　　要：慢性肝病是慢性肝损伤所致病理表现的统称,疾病早期若不及时治疗会衍变为肝硬化,甚至肝癌,给患者生活质量造成影响。仝小林认为慢性肝病的基本病理变化为毒瘀作祟、阻滞于肝络,其核心态为“肝毒”,临床常用熊胆粉、土茯苓、鬼箭羽三味小方治疗“肝毒”。方中熊胆粉保肝清热解毒,常用剂量为0.25~0.5 g;土茯苓除湿解毒,常用剂量为30~60 g,最大用量可至240 g,视其湿毒程度而定;鬼箭羽破血通经解毒,常用剂量为15~45 g。Chronic liver disease refers to a range of pathological conditions resulting from prolonged liver injury.If left untreated in the early stages,this disease may progress to cirrhosis or even liver cancer,significantly impacting patients'quality of life.Professor Tong Xiaolin posits that the fundamental pathological changes associated with chronic liver disease are characterized by the accumulation of toxins and blood stasis,which obstruct the liver meridian.He recognizes tts typical state as"hepatotoxicity",and in clinical practice he commonly employs a three-ingredient formula of bear bile powder,glabrous greenbrier rhizome and ramuli euonymi to address the condition.Among the three drugs,bear bile powder is used to protect the liver,clear heat and detoxify,with a commonly used dosage of 0.25-0.5g,and the common dosage of glabrous greenbrier rhizome is 30-60 g for dehumidification and detoxification,with a maximum dosage of 240 g depending on the degree of dampness and toxicity,while ramuli euonymi can expel blood stasis and dredge the meridians and detoxify,with a commonly used dosage of 15-45g.
