温阳法治疗岭南地区慢性胃炎经验

摘　　要：慢性胃炎发病机制与先天禀赋及后天环境等多种因素有关。岭南地区气候炎热,民众过服寒凉、久居寒湿之地,素体脾胃虚弱,导致中焦脾胃气机升降失调,出现湿热、寒湿等病理产物。岭南以寒湿之邪为患,寒性收引,湿性黏滞,故久病不愈、筋脉拘急,是岭南地区慢性胃炎发病关键。岭南地区慢性胃炎的治疗当尊仲景学说,重视温阳之法,提倡温阳散拘急、重视顾护阳气,以甘草干姜汤为基础,组方时活用附子、干姜等温阳之品,结合患者情志、饮食习惯、生活环境等多因素,辨病辨证用药,方可获良效。Chronic gastritis is a common and frequently occurring clinical condition,whose pathogenesis is related to various factors including a person's congenital endowment and postnatal environmental factors.In the Lingnan region,where the climate is hot and humid,the local population tends to overconsume cold and uncooked foods,and often resides in cold and damp environments.This leads to a constitutional deficiency of the spleen and stomach,making them more susceptible to external pathogenic factors.As a result,disharmony in the ascending and descending movements of qi in the middle burner can lead to the formation of pathological products such as dampness-heat and cold-dampness.In the context of multiple pathogenic factors,cold-dampness is particularly prevalent in the Lingnan region.The cold nature of pathogenic coldness causes contraction,while the sticky nature of dampness leads to lingering illness,resulting in spasm in the sinews and vessels,which is a key factor for the onset of chronic gastritis in the Lingnan area.Helicobacter pylori(Hp)infection plays a significant role in the occurrence and development of chronic gastritis.Western medicine commonly employs antibiotics for its treatment,which are considered cold in nature and can exacerbate the spleen and stomach's deficiency.In the diagnosis of chronic gastritis,modern equipment can serve as an effective extension of the traditional four diagnostic methods of Chinese medicine.After a definitive diagnosis,the treatment of chronic gastritis in the Lingnan region should adhere to Zhang Zhongjing's medical theories,emphasizing the importance of warming yang.The approach focuses on removing spasm and prioritizes the protection of yang qi,targeting the fundamental pathomechanism of spleen and stomach deficiency with coldness.The treatment employs warming yang methods,using licorice root and dry ginger decoction as the basis,with the flexible application of warming herbs such as monkshood,dry ginger.Simultaneously,the treatment considers patients'emotional states,
