摘　　要：目的观察推拿联合加味黄芪赤风汤治疗神经根型颈椎病气虚血瘀证患者的临床疗效。方法选择神经根型颈椎病气虚血瘀证患者120例,按随机数表法分为对照组与观察组,各60例。对照组予加味黄芪赤风汤治疗,观察组在对照组的基础上联合推拿治疗。比较2组治疗前后疼痛视觉模拟评分(VAS)、颈椎功能障碍指数(NDI)评分、日本骨科协会(JOA)评分、颈椎前屈活动度、中医症状积分、椎-基底动脉血流参数以及治疗后6个月生活质量量表(SF-36)评分变化情况。结果治疗后,观察组VAS评分、NDI评分、中医症状积分低于对照组(P<0.05),JOA评分、颈椎生理曲度明显高于对照组(P<0.05),RI低于对照组(P<0.05),PSV与TMFV高于对照组(P<0.05)。治疗后6个月,观察组SF-36评分高于对照组(P<0.05)。结论推拿联合加味黄芪赤风汤治疗神经根型颈椎病气虚血瘀证患者,可提高临床疗效,改善患者颈椎功能,促进患者椎-基底动脉血流供应恢复。Objective To observe the effect of massage combined with Huangqi Chifeng Decoction in the treatment of cervical spondylotic radiculopathy with the syndrome of blood stasis due to qi deficiency.Methods A total of 120 cervical spondylotic radiculopathy patients with the syndrome of blood stasis due to qi deficiency,treated in the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from May 2020 to May 2023,were randomly assigned to either the control group(n=60),which received modified Huangqi Chifeng Decoction,or the combination group(n=60),which received both the decoction and tuina therapy.The outcomes measured included the scores of the Visual Analog Scale(VAS)for pain,the Neck Disability Index(NDI),the Japanese Orthopaedic Association(JOA),cervicalflexion mobility,traditional Chinese medicine symptoms,vertebral-basilar arterial bloodflow parameters,and quality of life scale(SF-36)after six months of treatment.Results After treatment,the combination group showed lower VAS and NDI scores and TCM symptom scores than those in the control group(P<0.05).Additionally,the JOA scores and cervical spine curvature were significantly higher in the combination group(P<0.05),and the RI scores were lower,while PSV and TMFV were higher in the combination group compared to the control group(P<0.05).After 6 months of treatment,the SF-36 scores in the combination group were higher than those in the control group(P<0.05).Conclusion Massage combined with modified Huangqi Chifeng Decoction can enhance clinical outcomes,improve cervical function,and promote vertebral-basilar arterial bloodflow in cervical spondylotic radiculopathy patients with the syndrome of blood stasis due to qi deficiency.
