非增殖期糖尿病视网膜病变患者应用长春胺缓释胶囊联合前列地尔注射液治疗的效果观察

摘　　要：目的 分析非增殖期糖尿病视网膜病变(NPDR)患者应用长春胺缓释胶囊联合前列地尔注射液治疗的效果。方法 选取2022年1月至2024年4月周口市中医院诊疗的NPDR患者97例,根据治疗方案不同分为对照组、联合组。其中予以前列地尔注射液的49例为对照组,予以长春胺缓释胶囊联合前列地尔注射液的48例为联合组。比较两组临床疗效,不良反应发生率和治疗前、治疗3个月后眼动脉血流动力学[搏动指数(PI)、动脉舒张末期容积(Edv)、阻力指数(RI)、收缩期峰值血流速度(Psv)],眼部体征[视野灰度值、血管瘤数量、出血斑面积、黄斑中心凹厚度(CMT)],相关血清因子[白介素(IL)-2、肿瘤坏死因子(TNF)-α、转化生长因子(TGF)-β1、胱抑素(Cys-C)]水平。结果 联合组临床总有效率91.67%高于对照组75.51%(P<0.05)。治疗3个月后,联合组PI、RI水平降低幅度大于对照组,Psv、Edv水平升高幅度大于对照组(P<0.05);联合组出血斑面积、视野灰度值、CMT、血管瘤数量水平低于对照组(P<0.05);联合组IL-2、TNF-α、TGF-β1、Cys-C水平低于对照组(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 长春胺缓释胶囊联合前列地尔注射液治疗NPDR疗效显著,不仅可调节眼动脉血流动力学,改善眼部体征,还可抑制体内炎症反应,并具有一定安全性。Objective To analyze the effect of treatment with vinpocetine sustained-release capsules combined with alprostadil injection in patients with non-proliferative diabetic retinopathy(NPDR).Methods A total of 97 patients with NPDR who were treated in Zhoukou Traditional Chinese Medicine Hospital from January 2022 to April 2024 were selected and divided into a control group and a combination group based on different treatment regimens.Among them,49 patients who received alprostadil injection alone were assigned to the control group,while 48 patients who received a combination of vinpocetine sustained-release capsules and alprostadil injection were assigned to the combination group.The clinical efficacy and incidence of adverse reactions were compared between the two groups,as well as the changes in ocular arterial hemodynamics[pulsatility index(PI),end-diastolic volume(Edv),resistance index(RI),peak systolic velocity(Psv)],ocular signs[visual field grayscale value,number of hemangiomas,area of hemorrhagic spots,central macular thickness(CMT)],and levels of related serum factors[interleukin(IL)-2,tumor necrosis factor(TNF)-α,transforming growth factor(TGF)-β1,cystatin(Cys-C)]before and after 3 months of treatment.Results The total clinical effective rate in the combination group was 91.67%,which was higher than that in the control group(75.51%)(P<0.05).After 3 months of treatment,the decrease in PI and RI levels in the combination group was greater than that in the control group,while the increase in Psv and Edv levels were greater than those in the control group(P<0.05);the hemorrhagic spot area,visual field grayscale value,CMT,and hemangioma number levels in the combination group were lower than those in the control group(P<0.05);the levels of IL-2,TNF-α,TGF-β1,and Cys-C in the combination group were lower than those in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion The combination therapy of vinpocetine sustained-rele
