下肢深静脉血栓形成患者经抵当通脉汤联合利伐沙班治疗的效果分析

摘　　要：目的 回顾性分析抵当通脉汤联合利伐沙班治疗下肢深静脉血栓形成(DVT)患者的临床效果。方法 收集漯河市第二人民医院2022年5月至2024年4月118例下肢DVT患者,1∶1匹配设计根据治疗方法不同分为联合组(n=59)、对照组(n=59),对照组给予利伐沙班治疗,联合组在此基础上加用抵当通脉汤。对比两组疗效、中医证候积分、肿胀情况、凝血指标、血液流变学指标。结果 联合组总有效率(98.31%)高于对照组(86.44%)(P<0.05)。治疗后,联合组患肢广泛性肿胀、胀痛,皮肤暗红色,浅静脉怒张,发热,渴不欲饮等中医证候积分低于对照组(P<0.05);联合组腓中段、股中段周径小于对照组(P<0.05);联合组纤维蛋白原(FIB)、血浆黏度、全血高切黏度、全血低切黏度水平低于对照组,凝血酶时间(TT)、活化部分凝血活酶时间(APTT)、凝血酶原时间(PT)水平高于对照组(P<0.05)。结论 抵当通脉汤联合利伐沙班治疗下肢DVT疗效确切,可改善血液高凝状态、血液流变学及患肢肿胀情况。Objective To retrospectively analyze the clinical efficacy of didang tongmai decoction combined with rivaroxaban in the treatment of patients with deep venous thrombosis(DVT)in the lower limbs.Methods A total of 118 patients with lower limbs DVT from May 2022 to April 2024 in Luohe Second People's Hospital were collected.A 1∶1 matching design was used to divide the patients into a combined group(n=59)and a control group(n=59)based on different treatment methods.The control group was treated with rivaroxaban,while the combined group was given didang tongmai decoction on this basis.The efficacy,TCM syndrome score,swelling,coagulation indicators,and hemorheology indicators were compared between the two groups.Results The total effective rate of the combined group(98.31%)was higher than that of the control group(86.44%)(P<0.05).After treatment,the scores of TCM syndromes such as extensive swelling,pain,dark red skin,superficial vein engorgement,fever,and thirst in the combined group were lower than those in the control group(P<0.05);the circumference of the middle segment of the fibula and the middle segment of the thigh in the combined group were smaller than those in the control group(P<0.05);the levels of fibrinogen(FIB),plasma viscosity,whole blood high shear viscosity,and whole blood low shear viscosity in the combined group were lower than those in the control group,while the levels of thrombin time(TT),activated partial thromboplastin Time(APTT),and prothrombin time(PT)were higher than those in the control group(P<0.05).Conclusion The treatment of DVT in the lower limbs with didang tongmai decoction combined with rivaroxaban has a definite therapeutic effect,which can improve the hypercoagulable state of blood,hemorheology,and swelling of the affected limb.
