摘　　要：目的:探索多模态MR成像在评估直肠癌新辅助治疗后磁共振肿瘤退缩分级(Magnetic Resonance Im-aging Tumor Regression Grade,mrTRG)的价值。方法:纳入2018年1月至2023年7月在我院住院的局部进展期直肠癌患者55例,分析患者临床资料,应用不同序列组合评价mrTRG分级,记录相关结果,采用Kappa一致性检验进行评价。结果:高分辨率T2WI(HR-T2WI)联合DWI评估直肠癌新辅助治疗后mrTRG分级的准确率为69.1%,高估4例,低估13例,Kappa值为0.545;HR-T2WI、DWI联合动态增强磁共振(Dynamic Contrast-Enhanced Magnetic Resonance,DCE-MR)评估直肠癌新辅助治疗后mrTRG分期的准确率为83.63%,高估4例,低估4例,Kappa值为0.759。结论:HR-T2WI、DWI联合DCE-MR评估直肠癌新辅助治疗后mrTRG分级的准确性高于HR-T2WI联合DWI,可为临床制定下一步诊疗计划提供帮助。Objective:To explore the value of multimodal MR in the evaluation of magnetic resonance imaging tumor regression scale(mrTRG)after neoadjuvant chemoradiotherapy.Methods:55 patients with locally advan-ced rectal cancer hospitalized in our hospital from January 2018 to July 2023 were included,the clinical data of patients were analyzed,mrTRG grade was evaluated by different sequence combinations,relevant results were recorded,and evaluated by Kappa consistency test.Results:Among the included patients,33 males and 22 females;High-resolution T2WI(HR-T2WI)combined with DWI was 69.1%accurate after neoadjuvant chemoradiotherapy grading of rectal cancer.4 overestimated and 13 underestimated,with a Kappa value of 0.545.The accuracy of HR-T2WI and DWI combined with dynamic contrast-enhanced magnetic resonance(DCE-MR)was 83.63%in assessing mrTRG stage after neoadjuvant chemoradiotherapy in rectal cancer.Overoverestimated 4 and underestimated 4,with a Kappa value of 0.759.Conclusion:With HR-T2WI,DWI and DCE,the accuracy of mrTRG grade after neoadjuvant chemoradiotherapy is higher than that of HR-T2WI and DWI,which can help the next diagnosis and treatment plan.
