自我调节疲劳在乳腺癌化疗患者心理弹性与积极度间的中介作用

摘　　要：目的探讨自我调节疲劳在乳腺癌化疗患者心理弹性与积极度间的中介作用。方法抽取2024年1~4月四川省2所三级甲等医院正在接受化疗的317例乳腺癌患者,采用自制的一般资料调查问卷、心理弹性量表、自我调节疲劳量表和患者积极度量表展开调查。Pearson相关分析心理弹性、自我调节疲劳、积极度之间的关系。利用Amos 26.0软件建立结构方程模型,Bootstrap进行中介效应检验。结果乳腺癌化疗患者心理弹性、自我调节疲劳、积极度总分分别为(62.20±10.53)分、(50.99±7.69)分、(57.46±8.92)分。积极度得分在年龄、文化程度、月收入、付费方式、疾病分期、确诊时长、化疗次数、化疗合并症种数分布方面有差异(P<0.001)。心理弹性与积极度呈正相关(P<0.01),自我调节疲劳与积极度呈负相关(P<0.01),心理弹性与自我调节疲劳呈负相关(P<0.01)。中介效应分析结果显示,自我调节疲劳在心理弹性与积极度之间具有中介效应,中介效应占总效应的48.654%。结论乳腺癌化疗患者积极度处于第三级水平,自我调节疲劳在患者心理弹性与积极度之间起中介作用,提示医护人员应采取措施改善患者心理弹性和自我调节疲劳,从而提升积极度。Objective To explore the mediating role of self-regulatory fatigue between psychological resilience and activation in breast cancer patients with chemotherapy.Methods Breast cancer patients with chemotherapy from January to April 2024 were investigated by using self-made general data questionnaire,Connor and Davidson’s Resilience Scale,Self-Regulatory Fatigue Scale and Patient Activation Measure.Results The score of psychological resilience,self-regulatory fatigue and patient activation was(62.20±10.53),(50.99±7.69),(57.46±8.92)respectively.Psychological resilience was positively correlated with patient activation(P<0.01),self-regulatory fatigue was negatively correlated with patient activation(P<0.01),psychological resilience was negatively correlated with self-regulatory fatigue(P<0.01).Self-regulatory fatigue had mediating effect between psychological resilience and patient activation,and the mediating effect accounted for 48.654% of the total effect.Conclusion The activation of breast cancer patients with chemotherapy is at the third level,and self-regulatory fatigue plays a mediating role between psychological resilience and patient activation.It is suggested that the medical staff should improve the psychological resilience and self-regulatory fatigue of patients,so as to improve the patient activation degree.
