CO_(2)点阵激光联合A型肉毒毒素治疗凹陷性瘢痕的疗效和安全性研究

摘　　要：目的研究CO_(2)点阵激光联合注射用A型肉毒毒素治疗凹陷性瘢痕的临床疗效和安全性。方法前瞻性研究2021年5月至2023年5月,凹陷性瘢痕患者120例,随机分为研究组和对照组,每组患者60例,两组患者均采取CO_(2)点阵激光治疗,研究组患者激光治疗后使用注射用A型肉毒毒素联合治疗;对照组仅行激光治疗。记录两组患者治疗后1 d、治疗后1个月和治疗后6个月的ECCA评分、临床疗效、瘢痕症状、生活质量以及不良反应情况。结果治疗后6个月,对照组和研究组患者治疗总有效率分为86.7%、96.7%,差异具有统计学意义(P<0.05)。治疗后1 d、治疗后1个月和治疗后6个月研究组ECCA评分显著低于对照组(P<0.05);两组患者的DLQI各指标均低于治疗前,且研究组患者的DLQI各指标评分显著低于对照组(P<0.05);两组患者的不良反应发生率比较,差异无统计学意义(P>0.05)。结论CO_(2)点阵激光联合注射用A型肉毒毒素治疗凹陷性瘢痕疗效较好,安全性较高。objective To study the clinical efficacy and safety to treat depressed scars jointly with CO_(2) dot matrix laser and botulinum toxin type A.Methods In this prospective study,totally 120 patients with depressed scars treated between May 2021 and May 2023 were selected and randomly divided into a study group and a control group,60 patients in each group.While the patients in both groups being given CO_(2) dot matrix laser treatment,the patients in the study group were applied botulinum toxin type A after the laser treatment.For both groups,the ECCA scores,clinical efficacy,scar symptoms,quality of life and adverse reactions were recorded 1 d,1 month and 6months after the treatment respectively.Results Six months after the treatment,the total effective rate of the control group and study group was 86.7% and 96.7% respectively,and the difference was statistically significant(P<0.05).The ECCA score was significantly lower in the study group than in the control group 1 day,1 month and 6 months after the treatment(P<0.05).The scores of DLQI indexes of both groups dropped after the treatment,and the post-treatment scores of DLQI indexes were much lower in the study group than in the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Featuring high patients'satisfaction and safety,the combination of CO_(2) dot matrix laser and Botox is effective in treating depressed scars.
