摘  要:目的观察老年慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者外周血中表达细胞因子信号传导蛋白抑制因子1(suppressor of cytokine signaling 1,SOCS1)对辅助性T细胞(helper T cell,Th)17、调节性T细胞(regulatory T cell,Treg)及其相关细胞因子的影响,探究两者的相关性及平衡关系。方法选取2018年1月至2022年12月在南京大学医院附属鼓楼医院体检或就诊的老年COPD患者61例,选取同期老年无COPD体检者61例为对照组,COPD患者按照GOLD分级标准分为1~4级,采用流式细胞术检测外周血Th17细胞和Treg细胞水平并计算比值;采用酶联免疫吸附法测定血清Th17和Treg相关细胞因子白细胞介素(interleukin,IL)-17、肿瘤坏死因子(tumor necrosis factor,TNF)-α、转化生长因子(transforming growth factor,TGF)-β1的水平。评估SOCS1与Th17/treg、炎性细胞因子的相关性。结果2组参与者年龄、性别、体质量指数等基线资料差异无统计学意义(P>0.05);与对照组相比,COPD组IL-17、TNF-α、Th17/Treg水平高于对照组,SOCS1水平、TGF-β1水平低于对照组,差异有统计学意义[pg/mL:(64.58±9.54)比(47.87±7.69),(125.41±32.56)比(73.27±22.73),(1.25±0.24)比(1.25±0.24);ng/mL:(0.72±0.23)比(1.48±0.41);pg/mL:(682.06±117.70)比(879.51±145.37),t值分别为10.65,10.25,10.58,12.63,8.24,P值均<0.05];随着GOLD等级升高,COPD患者外周血IL-17、TNF-α、Th17/Treg水平均明显升高,SOCS1、TGF-β1水平明显降低,差异有统计学意义[pg/mL:(49.15±5.26)比(56.71±5.03)比(64.63±5.87)比(67.92±6.12);(86.45±24.83)比(107.16±26.24)比(125.53±28.41)比(138.27±30.10);(0.68±0.15)比(0.89±0.26)比(1.25±0.31)比(1.48±0.54);ng/mL:(1.75±0.55)比(1.47±0.52)比(1.13±0.39)比(0.78±0.18);pg/mL:(902.15±164.23)比(825.47±145.52)比(647.29±133.60)比(586.58±118.36),F值分别为30.90,8.28,20.20,11.52,12.64,P值均<0.05];Pearson相关分析显示,COPD组外周血Th17/Treg水平与IL-17、TNF-α均呈正相关关系,与SOCS1Objective To observe the effect of suppressor of cytokine signaling 1(SOCS1)expression in the peripheral blood of elderly patients with chronic obstructive pulmonary disease(COPD)on helper T cell(Th)17,regulatory T cells(Treg)and their related cytokines,and to investigate the correlation and balance between them.Methods Total of 61 cases of elderly COPD patients who were physically examined or visited in Nanjing Drum Tower Hospital from January 2018 to December 2022 were selected,and 61 elderly non-COPD physical examiners in the same period were selected as the control group.COPD patients were classified into grades 1-4 according to GOLD grading criteria,and the ratio of Th17 cells to Treg cells in the peripheral blood of all patients was measured by flow cytometry.The concentrations of serum Th17 and Treg-related cytokines interleukin(IL)-17,tumor necrosis factor(TNF)-α,and transforming growth factor(TGF)-β1 were measured by enzyme-linked immunosorbent assay.The correlation between SOCS1 and Th17/Treg,and inflammatory cytokines was assessed.Results There was no significant difference in the baseline data of age,gender,and body mass index of the participants in the two groups(P>0.05);compared with the control group,the levels of IL-17,TNF-α,and Th17/Treg in the COPD group were significantly higher than those in the control group,while the levels of SOCS1 and TGF-β1 were significantly lower than those in the control group,and the differences were statistically significant[pg/mL:(64.58±9.54)vs(47.87±7.69);(125.41±32.56)vs(73.27±22.73);(1.25±0.24)vs(1.25±0.24);ng/mL:(0.72±0.23)vs(1.48±0.41);pg/mL:(682.06±117.70)vs(879.51±145.37),t values were 10.65,10.25,10.58,12.63,8.24,all P values<0.05];As the GOLD level increases,,the levels of IL-17,TNF-α and Th17/Treg were markedly increased in the COPD group,and the levels of SOCS1 and TGF-β1 were decreased in the COPD group[pg/mL:(49.15±5.26)vs(56.71±5.03)vs(64.63±5.87)vs(67.92±6.12);(86.45±24.83)vs(107.16±26.24)vs(125.53±28.41)vs(138.27±30.10);(0.68�
