摘 要:目的探讨初发2型糖尿病(type 2 diabetes mellitus,T2DM)患者外周血白细胞介素(interleukin,IL)-35和IL-37表达水平及其与糖尿病相关指标的相关性。方法选取咸阳市第一人民医院内分泌科2021年6月至2023年1月收治的264例初发T2DM患者纳入病例组,同期选取250例体检健康人群纳入对照组,根据病例组干预3个月时的糖化血红蛋白(hemoglobinA1c,HbA1c)水平,将患者分为血糖控制较好组(201例)和血糖控制较差组(63例),采用双抗体夹心酶联免疫吸附试验测定患者外周血IL-35和IL-37水平;同时检测并比较病例组与健康组空腹血糖(fasting blood-glucose,FBG)、餐后2 h血糖(2-h postprandial blood glucose,2hPG),HbA1c,血脂谱及尿酸(uric acid,UA),空腹胰岛素(fasting insulin,FINS)水平,分析IL-35和IL-37水平及其与糖尿病相关指标的相关性。结果病例组与健康组性别、年龄、体质量指数、吸烟、饮酒、血脂指标及UA等差异无统计学意义(P>0.05);病例组FBG、2hPG、FINS、HbA1c、胰岛素抵抗指数(HOMA-IR)均高于对照组,IL-35和IL-37低于对照组(t值分别是8.20,4.34,10.71,8.96,12.84,11.19,5.78,P值均<0.05);血糖控制较好组的IL-35和IL-37水平低于血糖控制较差组[pg/mL:(70.35±10.31)比(88.19±15.24),(71.45±20.06)比(83.77±20.39),t值分别9.85和6.78,P值均<0.05];初发T2DM患者在是否合并冠心病时IL-35和IL-37水平发生明显变化,以合并冠心病的IL-35和IL-37水平更高(t值10.17和12.45,P值均<0.05),不同性别、年龄、身体质量指数(body mass index,BMI)及高血压的患者IL-35和IL-37水平差异无统计学意义(P>0.05);初发T2DM病患者IL-35和IL-37均与FBG、2hPG、FINS和HOMA-IR呈负相关,IL-35与IL-37水平呈正相关(P值均<0.05);而IL-35和IL-37与HbA1c、UA及其他血脂指标无明显相关性(P值均>0.05)。结论初发T2DM患者外周血清IL-35和IL-37降低,与FPG、2hPG、FINS和HOMA-IR呈负相关,IL-35与IL-37水平呈正相关,早期检测患者IL-35和IL-37表达对Objective To investigate the expression levels of interleukin(IL)-35 and IL-37 in peripheral blood of patients with type 2 diabetes(T2DM)and their correlation with diabetes related indicators.Methods A total of 264 patients with newly diagnosed T2DM admitted to the Department of Endocrinology,the First People's Hospital of Xianyang from June 2021 to January 2023 were selected as the case group,and 250 healthy people were selected as the control group during the same period.The patients were divided into well-controlled group(201 cases)and poorly-controlled group(63 cases).The levels of IL-35 and IL-37 were measured by double-antibody sandwich enzyme-linked immunosorbent assay.At the same time,fasting blood-glucose(FBG),2-h postprandial blood glucose(2hPG),hemoglobin A1c(HbA1c)were detected and compared between the case group and the healthy group.Blood lipid profile,uric acid(UA),fasting insulin(FINS)levels,IL-35 and IL-37 levels and their correlation with diabetes related indicators were analyzed.Results There were no significant differences in gender,age,body mass index(IBM),smoking,drinking,blood lipid indexes and UA between the case group and the healthy group(P>0.05).FBG,2 HPG and FINS,HbA1c,insulin resistance index(HOMA IR)in case group were higher than those in control group,the levels of IL-35,IL-37 is lower than the control group(t value are 8.20,4.34,10.71,8.96,12.84,11.19,5.78,all P values<0.05).The levels of IL-35 and IL-37 in well-controlled group were lower than those in poorly-controlled group[pg/mL:(70.35±10.31)vs(88.19±15.24),(71.45±20.06)vs(83.77±20.39),t value were 9.85 and 6.78,respectively,all P values<0.05].The levels of IL-35 and IL-37 were significantly changed in newly diagnosed T2DM patients with or without coronary heart disease.The levels of IL-35 and IL-37 were higher in patients with coronary heart disease(t=10.17 and 12.45,both P values<0.05).and hypertension patients had no significant difference in IL-35 and IL-37 levels(P>0.05).IL-35 and IL-37 were negatively correlated with
