摘 要:目的探讨系统免疫炎症指数(systemic immune-inflammation index,SII)在慢性子宫内膜炎(chronic endometritis,CE)病情评估及预后中的价值。方法选取宝鸡市妇幼保健院2021年1月至2023年11月行宫腔镜下子宫内膜活组织检查的CE患者244例,根据CD138免疫组织化学分析染色及宫腔镜检查结果分为CE组(152例),非CE组(92例)。测定并计算SII、酶联吸附法检测血管生成素-2(angiopoietin-2,Ang-2)及炎症因子[白细胞介素(interleukin-2,IL)-2、IL-6、IL-8、IL-10、肿瘤坏死因子(tumor necrosis factor,TNF)]-α水平、苏木精伊红染色检测CD138阳性率,对CE组和非CE组SII、IL-10、Ang-2、炎症因子、CD138进行比较。采用Pearson法分析SII与IL-10、Ang-2、CD138相关性,受试者工作特征(receiver operator characteristics,ROC)曲线分析SII、IL-10、Ang-2、CD138对子宫内膜炎的预测价值。结果IL-8、Ang-2 CE组低于非CE组,SII、CD138、IL-2、IL-4、IL-6、TNFα水平高于非CE组[ng/mL:(4.28±1.19)比(8.58±2.64),μg/mL:(0.67±0.07)比(1.19±0.12),%:(198.88±18.12)比(169.30±14.89),(1.38±0.14)×10^(9)/L比(1.38±0.14)×10^(9)/L,ng/mL:(3.94±0.37)比(1.68±0.33),(3.28±0.61)比(1.25±0.12),(2.67±0.30)比(1.58±0.24),(1.06±0.11)比(0.48±0.04),t值分别为17.39、42.77、13.19、53.19、48.13、31.53、29.58、48.63,P值均<0.05];Pearson相关性分析结果显示,SII与Ang-2呈负相关(r=-0.61,P<0.05)、SII与CD138呈正相关(r=0.62,P<0.05)、SII与IL-10呈负相关(r=-0.50,P<0.05),SII、IL-10、CD138、Ang-2联合检测ROC曲线下面积为0.958(95%CI:0.927~0.988)高于单一检测(P<0.05)。结论SII是CE病情评估和预后判断的重要指标,其升高与不良预后密切相关。Objective The study aims to explore the value of the systemic immune-inflammation index(SII)in the assessment and prognosis of chronic endometritis(CE).Methods A total of 244 patients with chronic endometritis who underwent hysteroscopic endometrial biopsy at Baoji Maternal and Child Health Hospital from January 2021 to November 2023 were selected.Based on CD138 immunohistochemical staining and hysteroscopic examination results,patients were divided into a chronic endometritis group(152 cases)and a non-CE group(92 cases).Measure and calculate the SII,and inflammatory factors[interleukin(IL)-2,IL-6,IL-8,IL-10,tumor necrosis factor(TNF)]-α were detected by the enzyme-linked immunosorbent assay(ELISA).CD138 positivity rate was assessed with hematoxylin-eosin staining.Comparisons of SII,IL-10,angiopoietin-2(Ang-2),inflammatory factors,and CD138 were made between the two groups.Pearson's correlation was used to analyze the relationship between SII and IL-10,Ang-2,CD138,and receiver operator characteristics(ROC)curve analysis was conducted to assess the predictive value of SII,IL-10,Ang-2,CD138 for endometritis.Results The levels of IL-8 and Ang-2 in the CE group were significantly lower than those in the non-CE group,while the levels of SII,CD138,IL-2,IL-4,IL-6,and TNF-α were significantly higher in the CE group compared to the non-CE group[ng/mL:(4.28±1.19)vs(8.58±2.64),μg/mL:(0.67±0.07)vs(1.19±0.12),%:(198.88±18.12)vs(169.30±14.89),(1.38±0.14)×10^(9)/L vs(1.38±0.14)×10^(9)/L,ng/mL:(3.94±0.37)vs(1.68±0.33),(3.28±0.61)vs(1.25±0.12),(2.67±0.30)vs(1.58±0.24),(1.06±0.11)vs(0.48±0.04),t values were 17.39,42.77,13.19,53.19,48.13,31.53,29.58 and 48.63,respectively,all P values<0.05].Pearson correlation analysis showed a negative correlation between SII and Ang-2(r=-0.61,P<0.05),a positive correlation between SII and CD138(r=0.62,P<0.05),and a negative correlation between SII and IL-10(r=-0.50,P<0.05).The ROC curve for combined detection of SII,IL-10,CD138,Ang-2 was 0.958(95%CI:0.927~0.988),which was hig
