针刺联合中药外敷治疗哺乳期急性乳腺炎的临床疗效

摘　　要：目的 观察哺乳期急性乳腺炎采用针刺联合中药外敷治疗的临床疗效。方法 选取2023年1月至2023年10月唐山市妇幼保健院120例哺乳期急性乳腺炎患者。将受试者根据就诊顺序分为治疗组(62例,于2023年6月至10月收治)和对照组(58例,于2023年1月至5月收治),对照组采取中药外敷治疗,治疗组在对照组基础上给予针刺治疗。观察两组患者临床疗效、治疗前后乳房局部情况(乳房肿块、乳汁分泌、乳房胀痛)评分、C反应蛋白(CRP)水平,统计体温恢复正常所用时间、乳房肿痛消失时间、通乳时间及肿块消失时间。结果 治疗组患者治疗总有效率高于对照组(96.77%vs 82.76%,χ^(2)=6.541,P<0.05)。治疗后两组患者乳房肿块、乳汁分泌、乳房胀痛评分低于治疗前,且治疗组低于对照组(P<0.05)。治疗后,治疗组血清CRP水平低于治疗前和对照组,差异有统计学意义(P<0.05)。治疗组患者体温恢复正常所用时间、乳房肿痛消失时间、通乳时间及肿块消失时间短于对照组(P<0.05)。结论 针刺联合中药外敷治疗哺乳期急性乳腺炎的疗效确切,可改善患者临床症状,降低患者炎症反应程度,缩短恢复时间。Objective To observe the clinical effect of acupuncture combined with external application of traditional Chinese medicine on acute mastitis during lactation.Methods A total of 120 patients with lactation acute mastitis admitted to Tangshan Maternal and Child Health Hospital from January to October 2023 were selected.Subjects were divided into treatment group(62 cases,admitted from June to October 2023)and control group(58 cases,admitted from January to May 2023)according to the order of visits.The control group was treated with external application of traditional Chinese medicine,and the treatment group was treated with acupuncture on the basis of the control group.Clinical efficacy,scores of local breast conditions(breast mass,milk secretion,breast tenderness)and C-reactive protein(CRP)levels before and after treatment were observed in the two groups.The time for body temperature to return to normal,the time for breast swelling and pain to disappear,the time for milk clearing and the time for tumor to disappear were analyzed.Results Compared with the control group,the total response rate was higher in the treatment group(96.77%vs 82.76%,χ^(2)=6.541,P<0.05).After treatment,the scores of breast mass,milk secretion,breast tenderness and other symptoms in the two groups were lower than before treatment(P<0.05),and the scores of the above indicators after treatment in treatment group were lower than those in the control group(P<0.05).After treatment,the serum CRP level in the treatment group was significantly lower than that in the pre-treatment and control groups,with statistical significances(P<0.05).The time of body temperature return to normal,the time of breast swelling pain disappearance,the time of milk clearing and the time of mass disappearance in the treatment group were shorter than those in the control group(P<0.05).Conclusion Acupuncture combined with external application of traditional Chinese medicine in the treatment of lactation acute mastitis is effective,can improve the clinical symptoms of patie
