机采血小板红细胞溢出的影响因素分析及针对性干预对策

摘  要:目的分析机采血小板(PLT)红细胞溢出(冲红)的影响因素,并提出针对性干预对策。方法回顾性分析2023年1月至2024年7月在商丘市中心血站开展血细胞分离机采集(机采)PLT的无偿献血者的基本资料及体检资料,统计机采PLT冲红情况后并分成冲红组和未冲红组,收集并比较2组一般资料及体检资料,针对差异指标采取Logistic回归模型分析,确定影响机采PLT冲红的主要因素,后提出针对性干预措施。结果总共1632例无偿献血者纳入研究,其中机采PLT期间发生冲红70例纳入冲红组,占比为4.29%;未发生冲红1562例纳入未冲红组,占比为95.71%。2组红细胞压积(HCT)、红细胞计数(RBC)、平均血红蛋白量(MCH)、平均红细胞体积(MCV)、平均血红蛋白浓度(MCHC)、乳糜血以及精神紧张比较差异有统计学意义(P<0.05),其中冲红组的HCT、RBC、乳糜血以及精神紧张比例高于未冲红组(P<0.05),MCH、MCV、MCHC水平低于未冲红组(P<0.05)。存在差异的指标采取多元Logistic回归模型分析,发现HCT、RBC、MCH、MCV、MCHC、乳糜血以及精神紧张均为机采PLT冲红的影响因素,其中HCT、RBC、乳糜血以及精神紧张为危险因素(OR值均>1),MCH、MCV、MCHC为保护因素(OR值均<1)。结论机采PLT冲红的影响因素较多,主要包含HCT、RBC、MCH、MCV、MCHC、乳糜血以及精神紧张,需要结合各项因素开展针对性干预,重视机采前的血液指标检查,并辅以合理心理干预等,以尽可能减少冲红现象发生。Objective To analyze the influencing factors of machine-recovered platelet(PLT)with red blood cell spillage(red flush),and put forward targeted intervention countermeasures.Methods The basic data and physical examination data of the non-reimbursed blood donors who carried out machine-recovered PLT in Shangqiu Central Blood Station from January 2023 to July 2024 were retrospectively analyzed,the situation of machine-recovered PLT redness was statistically analyzed and then divided into redness-recovered group and nonredness-recovered group.The general and physical examination data of the two groups were collected and compared,and then the difference indexes were analyzed by adopting the Logistic regression model to determine the main factors that affect the redness-recovered machine-recovered PLT and then put forward targeted intervention measures.Logistic regression model was used to analyze the differences in the indicators to determine the main factors affecting the red flush of PLT and then propose targeted interventions.Results A total of 1632cases of unpaid blood donors were included in the study,of which 70 cases of red flush during PLT were included in the red flush group,accounting for 4.29%,and 1562 cases of no red flush were included in the no red flush group,accounting for 95.71%.There were statistically significant differences between the two groups in terms of erythrocyte hematocrit(HCT),erythrocyte count(RBC),mean haemoglobin volume(MCH),mean corpuscular volume(MCV),mean corpuscular haemoglobin concentration(MCHC),coeliac blood,and stress(P<0.05),with the red flush group having a higher proportion of HCT,RBC,coeliac blood,and stress than the no red flush group(P<0.05).The levels of MCH,MCV and MCHC were lower than those in the group without red flush(P<0.05).Differences in indicators were analyzed by multivariate Logistic regression model,and it was found that HCT,RBC,MCH,MCV,MCHC,celiac blood and mental tension were all factors influencing the red flush of machine-administered PLT,of which HCT,RBC,
