血清25-(OH)D3与IL-37在COPD急性加重期患者中的变化及其与免疫功能的关系

摘  要:目的分析血清25羟基维生素D3[25-(OH)D3]与白细胞介素37(IL-37)在慢性阻塞性肺疾病(COPD)急性加重期(AECOPD)患者的变化及其与免疫功能的相关性。方法2020年10月—2023年10月期间,选取100例AECOPD患者作为AECOPD组,并选取COPD稳定期患者100例作为COPD稳定期组,比较两组血清25-(OH)D3、IL-37、免疫功能,治疗2个月后将AECOPD患者分为病情良好组73例和病情恶化组27例,比较两组血清25-(OH)D3、IL-37,并分析AECOPD患者血清25-(OH)D3、IL-37与免疫功能指标的相关性。结果与COPD稳定期组比较,AECOPD组25-(OH)D3、CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、Treg更低(P<0.05),AECOPD组IL-37、CD8^(+)、Th17、Th17/Treg更高(P<0.05)。与病情恶化组比较,病情好转组25-(OH)D3更高(P<0.05),病情好转组IL-37更低(P<0.05)。经Pearson分析,AECOPD患者血清25-(OH)D3与CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、Treg呈正相关(P<0.05),与CD8^(+)、Th17、Th17/Treg呈负相关(P<0.05),AECOPD患者血清IL-37与CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、Treg呈负相关(P<0.05),与CD8^(+)、Th17、Th17/Treg呈正相关(P<0.05)。结论AECOPD患者血清25-(OH)D3降低,血清IL-37升高,免疫功能失衡,且AECOPD患者血清25-(OH)D3、IL-37与免疫功能密切相关。Objective To analyze serum 25 hydroxyvitamin D3[25-(OH)D3]and interleukin 37(IL-37)in patients with acute exacerbations of COPD(AECOPD)and its correlation with immune function.Methods From October 2020 to October 2023,100 AECOPD patients were selected as AECOPD group,and 100 patients with stable COPD were selected as the stable COPD group,the serum 25-(OH)D3,IL-37,immune function of the two groups were compared.After 2 months of treatment,AECOPD patients were divided into a improving group of 73 cases and a worsening group of 27 cases.The serum levels of 25-(OH)D3 and IL-37 in the two groups were compared,And the correlation between 25-(OH)D3,IL-37 and immune function were analyzed.Results Compared with the stable COPD group,AECOPD group 25-(OH)D3 CD3^(+),CD4^(+),CD4^(+)/CD8^(+),Treg were lower(P<0.05),IL-37,CD8^(+),Th17,and Th17/Treg were higher(P<0.05).Compared with the worsening group,the improving group had a higher 25-(OH)D3(P<0.05),IL-37 was lower(P<0.05).According to Pearson's analysis,Serum 25-(OH)D3 and CD3^(+),CD4^(+),CD4^(+)/CD8^(+),Treg were positively correlated in AECOPD patients(P<0.05);with CD8^(+),Th17 and Th17/Treg showed a negative correlation(P<0.05);Serum IL-37 and CD3^(+)CD4^(+),CD4^(+)/CD8^(+),Treg were negatively correlated(P<0.05),with CD8^(+)Th17 and Th17/Treg were positively correlated(P<0.05).Conclusion AECOPD patients have a decrease in serum 25-(OH)D3,an increase in serum IL-37,an imbalance in immune function,and serum 25-(OH)D3 and IL-37 are closely related to immune function.
