血清syndecan-1、LRG1、PGC-1α对重度烧伤患者急性肾损伤的早期预测价值

摘  要:目的分析血清蛋白聚糖-1(syndecan-1)、富亮氨酸α-2-糖蛋白1(LRG1)、过氧化物酶体增殖物激活受体γ辅激活因子1α(PGC-1α)对重度烧伤患者急性肾损伤(AKI)的早期预测价值。方法收集本院2021年6月—2023年6月就诊的127例重度烧伤患者为研究组,根据患者烧伤一周内是否出现AKI分为AKI组(56例)和非AKI组(71例)。另选取在本院体检的102名健康者作为健康组。Pearson法进行相关性分析;Logistic回归法分析影响因素;ROC曲线评估血清syndecan-1、LRG1、PGC-1α水平对重度烧伤患者发生AKI的早期预测价值。结果与健康组相比,研究组血清syndecan-1、LRG1水平升高,PGC-1α水平降低(P<0.05)。与非AKI组相比,AKI组APACHEⅡ评分、SOFA评分及血清syndecan-1、LRG1水平均升高,PGC-1α水平降低(P<0.05)。重度烧伤AKI患者血清syndecan-1与LRG1水平呈正相关(r=0.463,P<0.001),血清syndecan-1、LRG1与PGC-1α水平均呈负相关(r=-0.512、-0.493,P<0.001)。APACHEⅡ评分、SOFA评分、血清syndecan-1、LRG1水平是重度烧伤患者发生AKI的危险因素,PGC-1α水平是保护因素(P<0.05)。血清syndecan-1、LRG1、PGC-1α三者联合预测重度烧伤患者发生AKI的AUC为0.970,优于syndecan-1、LRG1、PGC-1α单独预测。结论在重度烧伤AKI患者中血清syndecan-1、LRG1水平升高,PGC-1α水平降低,三者联合检测对重度烧伤患者发生AKI具有较高的临床预测价值。Objective To analyze the early prediction value of serum proteosan-1(syndecan-1),leucine-richα-2-glycoprotein-1(LRG1)and peroxisomal proliferator-activated receptorγ-coactivator 1α(PGC-1α)in acute kidney injury(AKI)in severe burn patients.Methods From June 2021 to June 2023,127 patients with severe burns who visited our hospital were regarded as the study group.They were separated into AKI group(56 cases)and non AKI group(71 cases)based on whether AKI occurred within one week of burn.Another 102 healthy individuals who underwent physical examination in our hospital were regarded as the health group.Pearson method was applied to analyze the correlation;Logistic regression was applied to analyze the influencing factors;ROC curve was applied to evaluate the early prediction value of serum syndecan-1,LRG1,and PGC-1αlevels for the occurrence of AKI in severely burned patients.Results Compared with the healthy group,the serum syndecan-1 and LRG1 levels in the study group increased,while PGC-1αlevel decreased(P<0.05).Compared with the non AKI group,the APACHE II score,SOFA score,and serum syndecan-1 and LRG1 levels in the AKI group increased,while PGC-1αlevel decreased(P<0.05).The serum syndecan-1 and LRG1 levels in severely burned AKI patients were positively correlated(r=0.463,P<0.001),while the serum syndecan-1,LRG1,and PGC-1αlevels were negatively correlated(r=-0.512,-0.493,P<0.001).APACHE II score,SOFA score,serum syndecan-1,LRG1 level were risk factors for AKI in severely burned patients,and serum PGC-1αlevel was a protective factor(P<0.05).The combined diagnosis of serum syndecan-1,LRG1,and PGC-1αfor AKI in severely burned patients had an AUC of 0.970,which was superior to the single diagnosis of syndecan-1,LRG1,and PGC-1α.Conclusion In severely burned AKI patients,serum syndecan-1 and LRG1 levels increase while PGC-1αlevel decreases.The combined detection of the three has high clinical prediction value for the occurrence of AKI in severely burned patients.
