公立医院行风管理核心要求现状、问题及对策研究

摘　　要：目的:选取国家层面颁布的与公立医院行风管理相关政策文件,分析国家对公立医院廉洁风险防控要点的情况,为行风管理制度建设顶层设计提供参考。方法:访问国家卫生健康委官方网站,以“行风”“廉洁”“作风”“纠正不正之风”“腐败”等为关键词检索其发布的相关政策文件,时间跨度为2013年1月1日~2024年5月31日;运用内容分析法对文件发布时间、发布单位、廉洁风险防控要点内容等进行量化分析。结果:共纳入29份政策文件进行研究,68.97%的文件由医政部门发布;廉洁风险防控要点主要包括医疗机构管理权力、医疗服务廉洁风险、医药购销领域行为和其他4个方面。结论:建议从顶层设计上完善制度建设和实施细则;在医疗服务廉洁风险防控方面发布实施细则和具体罚则;在医药购销领域加强部门联动,建立协同机制;在新兴领域如基因检测、外配处方等方面填补制度空缺。Objective:To analyze national policy documents related to conduct management in public hospitals,identifying key points in integrity risk prevention and control,and providing references for the top-level design of conduct management system construction.Methods:Policy documents published by the National Health Commission were retrieved using keywords such as"conduct""integrity""work style""rectification of improper practices"and"corruption"from January 1,2013,to May 31,2024.Content analysis was applied to quantify aspects such as publication time,issuing authorities,and key points in integrity risk prevention and control.Results:A total of 29 policy documents were included,with 68.97% issued by medical administration departments.Key integrity risk prevention areas included hospital management authority,integrity risks in medical services,pharmaceutical procurement and sales behaviors,and other related fields.Conclusion:It is recommended to enhance institutional construction and implementation guidelines from a top-level design perspective;issue detailed regulations and penalties for integrity risk prevention in medical services;strengthen interdepartmental collaboration in pharmaceutical procurement and sales;and address regulatory gaps in emerging fields such as genetic testing and outsourced prescriptions.
