当归补血汤合二妙散对普瑞巴林治疗糖尿病周围神经痛患者疗效的影响

摘　　要：目的评价当归补血汤合二妙散对普瑞巴林治疗糖尿病周围神经痛患者疗效的影响。方法本研究为双向队列研究。纳入2020年12月1日至2023年12月1日在陕西中医药大学附属医院内分泌二科就诊的糖尿病周围神经痛患者,年龄≥18岁,性别不限。根据治疗方案不同分为常规组和联合组。常规组患者接受普瑞巴林治疗,75 mg/次,2次/d,连续干预3个月;联合组在常规组治疗基础上,联合应用当归补血汤合二妙散200 ml/次,2次/d,连续干预3个月。采用配对t检验比较患者干预前后血清炎症因子、血液流变学及凝血指标水平的变化;采用两独立样本t检验比较两组患者上述各指标和不良反应发生情况;采用非参数秩和检验比较两组的疗效。结果最终纳入分析的糖尿病周围神经痛患者64例,常规组和联合组各32例。联合组疗效优于常规组(Z=-2.18,P=0.029),两组不良反应发生率差异无统计学意义(15.6%比21.9%,χ^(2)=0.41,P=0.522)。干预后,联合组血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、巨噬细胞移动抑制因子(MIF)、血浆黏度、血浆D-二聚体(D-D)和纤维蛋白原(Fib)均低于常规组(P均<0.05)。结论当归补血汤合二妙散可通过减轻局部炎症,改善血液高凝状态,优化普瑞巴林治疗糖尿病周围神经痛的疗效,且不增加不良反应。ObjectiveTo evaluate the effects of danggui buxue decoction and ermiao powder on the efficacy of pregabalin in the treatment of patients with diabetic peripheral neuralgia.MethodsThis study was a two-directional cohort study.Patients with diabetic peripheral neuropathic pain,aged≥18 years old,regardless of gender,were collected from Second Department of Endocrinology,Affiliated Hospital of Shaanxi University of Traditional Chinese Medicine,from December 1,2020,to December 1,2023.Patients were divided into the conventional group and the combined group based on different treatment plans.Patients received pregabalin 75 mg/time,twice a day,for 3 months in the conventional group.Patients received additional danggui buxue decoction and ermiao powder 200 ml/time,twice a day,for 3 months in the combined group.The levels of serum inflammatory factors,blood rheology,and coagulation indicators before and after the intervention were compared by paired t-test.All the indicators and adverse reaction incidence of the two groups were compared by two sample t-test.The efficacy was analyzed by the nonparameter rank sum test between two groups.ResultsA total of 64 patients were included in the final analysis,with 32 cases in each group.The efficacy of the combined group was superior to that of the conventional group(Z=-2.18,P=0.029),but no difference in the incidence of adverse reactions between the two groups(15.6%vs.21.9%,χ^(2)=0.41,P=0.522).After the intervention,the levels of serum interleukin-6(IL-6),tumor necrosis factor-α(TNF-α),macrophage migration inhibitory factor(MIF),plasma viscosity,plasma D-dimer(D-D),and fibrinogen(Fib)in the combined group were all lower than those in the conventional group(all P<0.05).ConclusionDanggui buxue decoction and ermiao powder can reduce inflammation,improve hypercoagulability,and optimize the efficacy of pregabalin for patients with diabetic peripheral neuralgia,without increasing adverse reactions.
