摘 要:目的通过16S rRNA序列分析宫腔菌群与剖宫产子宫切口瘢痕憩室的相关性。方法收集2022—2023年温州市人民医院收治的剖宫产术后发生子宫瘢痕憩室(CSD)的产妇30例为研究组。另选择同期剖宫产术后未发生CSD的产妇30例为对照组。通过16S rRNA高通量测序检测宫腔菌群,对比两组患者宫颈菌群的物种组成与丰度分析。利用Cytoscape软件可视化互作关系,并通过DAVID网站进行通路富集分析,揭示关键通路。结果两组患者的胎膜早破人数、产后感染人数、切口位置、子宫位置、体质量增加差异具有统计学意义(P<0.05),多因素Logistic回归分析显示,胎膜早破、产后感染、切口位置近宫颈口、子宫位置后位以及体质量增加较大是CSD的独立影响因素(P<0.05)。对照组的OTU和α多样性指数均高于研究组(P<0.05)。门级分类显示,两组样本的群落模式具有相似性,研究组中变形菌门的相对丰度显著增加,而不动杆菌、梭杆菌、氯弯曲菌、平面菌、疣状体、硝基螺旋菌和蓝藻的相对丰度在研究组中显著低于对照组(P<0.05)。在属水平上,研究组与对照组之间存在4个显著不同的属,其中研究组的不动杆菌、梭杆菌和嗜血杆菌的相对丰度显著低于对照组(P<0.05)。通过DAVID数据库进行通路富集分析,分别得到有统计学意义的通路395条、104条。发现这些靶点与基因表达调控、软骨细胞分化、对外源刺激反应、细胞增殖等通路密切相关。利用STRING网站构建蛋白互作网络,并识别出95个关键靶点。结论剖宫产子宫切口瘢痕憩室患者的宫腔菌群组成发生改变,其组成和功能及调控炎症反应与细胞增殖等方面紧密相关。Objective Correlation between uterine microbiota and scar diverticulum in cesarean section through 16S rRNA sequence analysis.Methods Collect 30 cases of postpartum women with uterine scar diverticulum(CSD)after cesarean section admitted to Wenzhou People’s Hospital from 2022 to 2023 as the research group.Another 30 postpartum women who did not develop CSD after cesarean section during the same period were selected as the control group.Using high-throughput sequencing of 16S rRNA to detect uterine microbiota,compare the species composition and abundance analysis of cervical microbiota between two groups of patients.Visualize the interaction relationship using Cytoscape software and conduct pathway enrichment analysis through the DAVID website to reveal key pathways.Results There was a statistically significant difference(P<0.05)in the number of cases of premature rupture of membranes,postpartum infections,incision location,uterine location,and increased body weight between the two groups of patients.Multivariate Logistic regression analysis showed that premature rupture of membranes,postpartum infections,incision location near the cervical opening,posterior uterine location,and significant increase in body weight were independent influencing factors of CSD(P<0.05).The OTU and alpha diversity index of the control group were higher than those of the study group(P<0.05).According to the phylum classification,the community patterns of the two groups of samples were similar.The relative abundance of Proteobacteria significantly increased in the study group,while the relative abundance of Acinetobacter,Clostridium,Chloroflexi,Planobacter,Verrucous,Nitrospira,and Cyanobacteria were significantly lower in the study group than in the control group(P<0.05).At the genus level,there were four significant differences between the study group and the control group,with the relative abundance of Acinetobacter,Clostridium,and Haemophilus in the study group significantly lower than that in the control group(P<0.05).Through pathw
