冷冻5年的自体卵母细胞复苏后与新鲜卵子经供精单精子显微注射并行PGT-A的临床结局分析

摘　　要：目的介绍一例40岁患者使用冷冻5年的10枚自体卵母细胞复苏后与23枚新鲜卵子经供精单精子显微注射并行胚胎植入前非整倍体检测(PGT-A)的病历,比较不同来源卵子的胚胎发育情况和妊娠结局。方法对中国人民解放军总医院第六医学中心首例使用冷冻5年的自体卵子和新鲜周期卵子行PGT-A的病历进行分析,比较不同卵子来源的胚胎发育情况及妊娠结局。结果复苏卵子与新鲜卵子相比,受精率(70.0%vs.72.7%)、卵裂率(100%vs.100%)、优胚率(57.1%vs.62.5%)无明显差异;移植复苏卵子来源的一枚整倍体囊胚未孕,移植新鲜卵子来源的一枚整倍体囊胚活产一名女孩。结论取卵当日未能取精而行卵子冷冻,复苏卵子受精后的胚胎发育可能不受影响,但移植仍需谨慎应用。Objective This paper describes a 40-year-old patient who was thawed with 10 autologous oocytes frozen for 5 years and 23 fresh oocytes via intracytoplasmic sperm injection(ICSI)of donor sperm and preimplantation genetic testing for aneuploidie(PGT-A),and compares the euploid blastocyst rate and pregnancy outcome of oocytes from different sources.Methods The medical records of PGT-A using the first autologous oocyte cryopreservation(OC)for 5 years and fresh cycle oocytes in the Sixth Medical Center of PLA General Hospital were analyzed.The fertilization rate,cleavage rate,Day 3 quality embryo rate,blastocyst formation rate,euploid blastocyst rate and pregnancy outcome of different oocyte sources were compared.Results There were no significant differences in fertilization rate(70.0%vs.72.7%),cleavage rate(100%vs.100%)and Day 3 quality embryo rate(57.1%vs.62.5%)between warmed oocytes and fresh oocytes.There was no pregnancy when a euploid blastocyst from a warmed oocyte was transplanted,and a girl was born alive when a euploid blastocyst from a fresh oocyte was transplanted.Conclusion The embryo development of the warmed oocytes after fertilization may not be affected by OC without sperm retrieval on the day of oocyte retrieval,but the transfer still needs to be used with caution.
