双侧内括约肌挑断加肛裂切除术治疗肛裂的临床效果研究

摘　　要：目的:探索双侧内括约肌挑断加肛裂切除术治疗肛裂的临床效果。方法:研究以2023年1月—2024年6月诸城市人民医院收治的126例肛裂患者作为研究样本,按照随机数字表法分为对照组(行肛裂切除术治疗)和观察组(行双侧内括约肌挑断加肛裂切除术治疗),各63例。观察临床各项指标变化。结果:观察组总有效率高于对照组(P=0.038);观察组术中出血量少于对照组,伤口愈合时间、住院时间短于对照组(P<0.001);治疗后,两组视觉模拟评分法、肛周肿胀程度、肛门功能评分降低,且观察组低于对照组(P<0.05);治疗后,两组便血症状、便秘症状积分和肛管静息压降低,舒张压力升高,且观察组优于对照组(P<0.05);观察组并发症发生率低于对照组(P=0.020)。结论:双侧内括约肌挑断加肛裂切除术治疗肛裂的效果显著,可有效减轻肛周疼痛、肿胀程度,改善临床相关症状及肛管静息压、舒张压力,有助于恢复患者肛门功能,减少并发症。Objective:To investigate the clinical efficacy of bilateral internal sphincterotomy combined with anal fissurectomy in the treatment of anal fissure.Methods:A total of 126 patients with anal fissure admitted to Zhucheng People's Hospital from January 2023 to June 2024 were selected as the study subjects and randomly divided into a control group(treated with anal fissurectomy)and an observation group(treated with bilateral internal sphincterotomy combined with anal fissurectomy),with 63 cases in each group.The changes in various clinical indicators were observed.Results:The total effective rate in the observation group was higher than that in the control group(P=0.038).The intraoperative blood loss in the observation group was less than that in the control group,and the wound healing time and hospital stay were shorter than those in the control group(P<0.001).After treatment,the Visual Analog Scale scores,perianal swelling degree,and anal function scores in both groups decreased,and the observation group was lower than the control group(P<0.05).After treatment,the scores for hematochezia and constipation symptoms,as well as the anal resting pressure,decreased,while the diastolic pressure increased,and the observation group was superior to the control group(P<0.05).The incidence of complications in the observation group was lower than that in the control group(P=0.020).Conclusion:Bilateral internal sphincterotomy combined with anal fissurectomy is effective in treating anal fissure,significantly reducing perianal pain and swelling,improving clinical symptoms,anal resting pressure,and diastolic pressure,and aiding in the recovery of anal function and reduction of complications.
