心电图定量参数与老年心力衰竭患者心功能的相关性及对主要不良心血管事件的预测价值

摘　　要：目的:分析心电图定量参数与老年心力衰竭(CF)患者心功能的相关性及对主要不良心血管事件(MACE)的预测价值。方法:选取2023年1—12月于临沂市中心医院就诊的老年CF患者125例为研究对象。根据纽约心脏病协会(NYHA)心功能分级将患者分为Ⅱ级组(n=39)、Ⅲ级组(n=56)和Ⅳ级组(n=30)。比较三组心电图定量参数(QRS电压、QRS时限、QTc间期),分析心电图定量参数和老年CF患者心功能的相关性。随访12个月后,根据患者是否出现MACE分为MACE组(n=25)、无MACE组(n=100),比较两组心电图定量参数,分析心电图定量参数与老年CF患者心功能的相关性,探讨心电图定量参数对老年CF患者并发MACE的预测价值。结果:Ⅳ级组QRS电压低于Ⅱ级组与Ⅲ级组,QRS时限与QTc间期长于Ⅱ级组与Ⅲ级组(P<0.05);Ⅲ级组QRS电压低于Ⅱ级组,QRS时限与QTc间期长于Ⅱ级组(P<0.05)。QRS电压与老年CF患者心功能呈正相关(P=0.003),QRS时限、QTc间期与老年CF患者心功能呈负相关(P<0.001)。MACE组QRS电压低于无MACE组,QRS时限、QTc间期长于无MACE组(P<0.001)。受试者工作特征曲线分析显示,QRS电压、QRS时限、QTc间期联合预测老年CF患者并发MACE的价值更高(AUC=0.888,P<0.001)。结论:心电图定量参数与老年CF患者心功能有关,对患者MACE发生具有预测价值,且联合检测的预测价值更高。Objective:To analyze the correlation between quantitative electrocardiogram(ECG)parameters and cardiac function in elderly patients with heart failure(HF)and their predictive value for major adverse cardiovascular events(MACE).Methods:A total of 125 elderly patients with HF admitted to Linyi Central Hospital from January to December 2023 were selected as the study subjects.Patients were divided into ClassⅡ(n=39),classⅢ(n=56),and ClassⅣ(n=30)groups according to the New York Heart Association(NYHA)classification of cardiac function.The quantitative ECG parameters(QRS voltage,QRS duration,QTc interval)were compared among the three groups,and the correlation between these parameters and cardiac function in elderly patients with HF was analyzed.After 12 months of follow-up,patients were divided into MACE group(n=25)and non-MACE group(n=100)based on the occurrence of MACE.Quantitative ECG parameters were compared between the two groups,and the correlation between these parameters and cardiac function in elderly patients with HF was analyzed.The predictive value of quantitative ECG parameters for MACE in elderly patients with HF was explored.Results:The QRS voltage in ClassⅣgroup was lower than that in ClassⅡand ClassⅢgroups,and the QRS duration and QTc interval were longer than those in ClassⅡand Class III groups(P<0.05).The QRS voltage in ClassⅢgroup was lower than that in ClassⅡgroup,and the QRS duration and QTc interval were longer than those in ClassⅡgroup(P<0.05).QRS voltage was positively correlated with cardiac function in elderly patients with HF(P=0.003),while QRS duration and QTc interval were negatively correlated with cardiac function in elderly patients with HF(P<0.001).The QRS voltage in MACE group was lower than that in non-MACE group,and the QRS duration and QTc interval were longer than those in non-MACE group(P<0.001).Receiver operating characteristic curve analysis showed that the combination of QRS voltage,QRS duration,and QTc interval had a higher predictive value for MACE in el
