热淋清颗粒联合阿莫西林克拉维酸钾治疗儿童输尿管支架管相关尿路感染的临床研究

摘　　要：目的探讨热淋清颗粒联合阿莫西林克拉维酸钾治疗儿童输尿管支架管相关尿路感染的临床疗效。方法选取2022年7月—2024年7月郑州大学附属儿童医院收治的行肾积水或肾、输尿管结石术后放置输尿管支架的80例尿路感染患儿,依据随机数字法分为对照组(40例)和治疗组(40例)。对照组患儿口服阿莫西林克拉维酸钾颗粒,3~9个月患儿20 mg/(kg·d),3次/d;9个月~2岁患儿20 mg/(kg·d),2次/d;2~6岁患儿,1袋/次,2次/d。在对照组基础上,治疗组患儿口服热淋清颗粒,4 g/次,3次/d。两组患儿用药7 d。观察两组患儿临床疗效,比较治疗前后两组患儿症状缓解时间,及C反应蛋白(CRP)、白细胞介素-6(IL-6)、降钙素原(PCT)和肿瘤坏死因子-α(TNF-α)水平。结果治疗后,治疗组总有效率(95.00%)明显高于对照组(77.50%,P<0.05)。治疗后,与对照组对比,治疗组退热、尿菌转阴时间、尿频、尿痛、排尿困难缓解时间明显缩短(P<0.05)。治疗后,两组患者PCT、IL-6、TNF-α、CRP水平均明显较低(P<0.05),且以治疗组治疗后的变化更为显著(P<0.05)。结论热淋清颗粒与阿莫西林克拉维酸钾协同治疗儿童输尿管支架管相关尿路感染,可提高临床疗效,缩短症状缓解时间,促使患儿机体的炎性因子减弱,使肾功能的恢复加快。Objective To investigate the efficacy of Relinqing Granules combined with amoxicillin clavulanate potassium in treatment of ureteral stent-related urinary tract infection.Methods Children(80 cases)with ureteral stent-related urinary tract infection in Children’s Hospital Affiliated to Zhengzhou University from July 2022 to July 2024 were randomly divided into control(40 cases)and treatment(40 cases)group based on different treatments.Children in the control group were po administered with Amoxicillin and Clavulanate Potassium Granules,children aged 3 to 9 months 20 mg/(kg·d),three times daily,children aged 9 months to 2 years 20 mg/(kg·d),twice daily,children aged 2 to 6 years,1 bag/time,twice daily.Children in the treatment group were po administered with Relinqing Granules based on the control group,4 g/time,three times daily.Children in two groups were treated for 7 d.After treatment,the clinical evaluations were evaluated,the symptom relief time,and the PCT,IL-6,TNF-αand CRP in two groups before and after treatment were compared.Results After treatment,the effective rate in the treatment group(95.00%)was significantly higher than that in the control group(77.50%,P<0.05).After treatment,compared with the control group,the time for fever reduction,urine bacteria to turn negative,and the time for relief of frequent frequency,dysuria,and dysuria in the treatment group were significantly shortened(P<0.05).After treatment,the levels of PCT,IL-6,TNF-α,and CRP were significantly lower in two groups(P<0.05),and the changes after treatment in the treatment group were more significant(P<0.05).Conclusion The synergistic treatment of Relinqing Granules with amoxicillin and clavulanate potassium can improve clinical efficacy,shorten symptom relief time,reduce inflammatory factors,accelerate renal function recover.
