探究游离植皮结合负压封闭引流技术治疗糖尿病足溃疡的疗效

摘　　要：目的探究糖尿病足溃疡患者临床治疗中采用游离植皮结合负压封闭引流技术的疗效。方法根据不同治疗方法将2021年1月—2024年9月泰兴市第二人民医院收治的156例糖尿病足溃疡患者分为对照组(n=78,常规清创、引流、敷药治疗)和研究组(n=78,游离植皮结合负压封闭引流技术治疗)。对比两组临床疗效、创面愈合情况、血清指标、足背动脉血流速度及神经传导速度。结果研究组临床疗效高于对照组,溃疡面愈合时间、完全上皮化时间及住院时间均短于对照组,差异均有统计学意义(P均<0.05)。研究组治疗后的血清基质金属蛋白酶-9、富含半胱氨酸的酸性分泌蛋白水平均高于对照组,足背动脉血流速度、腓肠神经运动神经传导速度及腓肠神经感觉神经传导速度均大于对照组,差异均有统计学意义(P均<0.05)。结论采用游离植皮、负压封闭引流技术联合方案治疗糖尿病足溃疡的效果较好,可加快足背动脉血流速度及神经传导速度,改善血清指标,促进创面愈合。Objective To explore the curative effect of free skin grafting combined with vacuum sealing drainage in the clinical treatment of diabetic foot ulcer patients.Methods According to different treatment methods,156 patients with diabetic foot ulcer admitted to the Second People's Hospital of Taixing from January 2021 to September 2024 were divided into control group(n=78,routine debridement,drainage and drug application)and study group(n=78,free skin grafting combined with vacuum sealing drainage).The clinical efficacy,wound healing,serum indexes,dorsa⁃lis pedis artery blood flow velocity and nerve conduction velocity were compared between the two groups.Results The clinical efficacy of the study group was higher than that of the control group,and the healing time of the ulcer surface,complete epithelialization time and hospitalization time were shorter than those of the control group,the differences were statistically significant(all P<0.05).After treatment,the levels of serum matrix metalloproteinase-9 and cysteine-rich acidic secretory protein in the study group were higher than those in the control group,and the blood flow velocity of dorsalis pedis artery,motor nerve conduction velocity of sural nerve and sensory nerve conduction ve⁃locity of sural nerve were higher than those in the control group,the differences were statistically significant(all P<0.05).Conclusion The combination of free skin grafting and vacuum sealing drainage is effective in the treatment of diabetic foot ulcer.It can accelerate the blood flow velocity and nerve conduction velocity of dorsal artery of foot,im⁃prove serum index and promote wound healing.
