达格列净片辅助治疗磺脲类失效2型糖尿病的临床有效性研究

摘　　要：目的分析使用达格列净片治疗磺脲类药物治疗失效的2型糖尿病(type 2 diabetes mellitus,T2DM)患者的临床治疗效果、胰岛素抵抗及血糖变化情况。方法按治疗方法的不同,将2022年6月—2024年6月江苏省宿迁市中医院诊治的108例磺脲类药物治疗失效的T2DM患者分为对照组(n=54,采用二甲双胍治疗)与研究组(n=54,在对照组基础上采用达格列净片辅治)。比较两组胰岛素抵抗指数、血糖水平以及临床治疗效果。结果组间治疗总有效率比较,研究组更高,差异有统计学意义(P<0.05)。治疗后,相比于对照组,研究组胰岛素抵抗指数、空腹血糖、餐后2 h血糖、糖化血红蛋白水平均更低,差异均有统计学意义(P均<0.05)。结论达格列净片辅治磺脲类药物治疗失效的T2DM,疗效确切、降糖作用明显,能改善患者胰岛素抵抗。Objective To analyze the clinical efficacy,insulin resistance and blood glucose changes of dapagliflozin tablets in the treatment of type 2 diabetes mellitus(T2DM)patients with sulfonylurea treatment failure.Methods Ac⁃cording to different treatment methods,a total of 108 T2DM patients with sulfonylurea treatment failure in Suqian Hos⁃pital of Traditional Chinese Medicine from June 2022 to June 2024 were divided into control group(n=54,treated with metformin)and study group(n=54,treated with dapagliflozin tablets on the basis of the control group).The insulin re⁃sistance index,blood glucose levels and clinical treatment effect were compared between the two groups.Results The total effective rate of treatment between the two groups was compared,the study group was higher,and the difference was statistically significant(P<0.05).After treatment,compared with the control group,the levels of insulin resistance index,fasting plasma glucose,2-hour postprandial plasma glucose and glycated hemoglobin A1c in the study group were lower,and the differences were statistically significant(all P<0.05).Conclusion Dapagliflozin tablets are effec⁃tive in the treatment of T2DM with sulfonylurea treatment failure.It has obvious hypoglycemic effect and can improve insulin resistance.
