视网膜静脉阻塞患者视乳头周围灌注改变的定量分析研究

摘　　要：目的 应用光学相干断层扫描血管成像(OCTA)分析视网膜静脉阻塞(RVO)患者的患眼及健眼的视乳头周围放射状毛细血管(RPC)血流密度以及视乳头周围神经纤维层(RNFL)厚度的改变。方法 观察性横断面研究,纳入了单眼RVO患者25例,其中BRVO患者16例,CRVO患者9例,纳入年龄性别匹配的健康志愿者30例任意一眼为对照组共30眼。用OCTA量化视乳头周围RNFL厚度以及RPC血流密度,组间进行比较。结果 RVO患眼的平均RPC血流密度较对照组显著降低,除NI子区外的7个子区均有统计学意义(P<0.05)。CRVO组患眼的平均RPC血流密度较BRVO组显著降低(t=4.11,P<0.05),且除SN子区外的7个子区的降低均具有统计学意义(P<0.05)。患眼的平均RNFL厚度高于对照眼,主要在TI及TS子区(P<0.05);CRVO患眼与BRVO患眼之间平均RNFL厚度无显著差异,但在IN及IT子区中变薄(P<0.05)。整体患者健眼的平均RPC血流密度与对照眼无显著差异,但CRVO患者健眼较BRVO健眼及对照组显著降低(t=2.55、-3.30,P<0.05),整体健眼的平均RNFL厚度无显著改变,但CRVO健眼较BRVO健眼及对照组变薄(t=2.12、-2.73,P<0.05)。结论RVO患眼RPC血流密度显著降低,RNFL厚度在部分区域增厚,CRVO患者变化更明显。患者健眼亦存在轻度血流密度及RNFL厚度的异常,提示RVO与全身系统疾病的相关性。ObjectiveTo analyzethechangesin radial peripapillary capillary(RPC)vessel density and peripapillary retinal nerve fiber layer(RNFL)thickness in the affected and fellow eyes of patients with retinal vein occlusion(RVO)using optical coherence tomography angiography(OCTA).Methods This observational cross-sectional study included 25 patients with unilateral RVO(16 with branch retinal vein occlusion BRVO and 9 with central retinal vein occlusion CRVO)and 30 age-and sex-matched healthy controls(30 eyes).OCTA was used to measure and quantify RPC vessel density and RNFL thickness around the optic disc,and comparisons were made between groups.Results A total of 25 RVO patients(16 BRVO and 9 CRVO)and 30 healthy controls were included.The mean peripapilary RPC vessel density in affected eyes was significantly lower than in control eyes,with reductions observed in 7 of the 8 subfields except for the nasal inferior(NI)subfield(P<0.05).CRVO eyes showed significantly lower mean RPC vessel density compared to BRVO eyes,with reductions in 7 of the 8 subfields except for the superior nasal(SN)subfield(t=4.11,P<0.05).The mean RNFL thickness in affected eyes was greater than in control eyes,with significant thickening in the temporal inferior(TI)and temporal superior(TS)subfields(P<0.05).No significant differences in mean RNFL thickness were observed between CRVO and BRVO eyes,although CRVO eyes exhibited significant thinning in the inferior nasal(IN)and inferiortemporal(IT)subfields(P<0.05).Inthefelloweyes,themeanRPC vessel density showed no significant dfference compared to control eyes.However,CRVO fellow eyes exhibited significantly lower RPC vessel density compared to both BRVO fellow eyes and control eyes(t=2.55、-3.30,P<0.05),ThemeanRNFL thicknessinfelloweyes showed no significant diferences compared to control eyes,but CRVO fellow eyes showed significant thinning compared to BRVO fellow eyes and control eyes(t=2.12、-2.73,P<0.05).Conclusion TheRPC vessel density in affected eyesof RVO patientswassignificantly reduced,while
