摘 要:目的分析支气管哮喘(BA)患儿血清微小核糖核酸(miR)-130a-3p、miR-326与气道炎症的关系及对预后的交互影响。方法选择2019年6月至2021年7月在本院接受治疗的120例BA患儿作为研究组,另选取同期在本院体检的120例健康者纳入对照组。采用实时荧光定量逆转录聚合酶链式反应(qRT-PCR)检测两组血清miR-130a-3p、miR-326水平,采用血细胞分析仪测定气道炎症因子[诱导痰嗜酸性粒细胞(EOS)]水平,经Pearson相关性分析血清miR-130a-3p、miR-326相对表达量与诱导痰EOS水平的相关性。随访,按照预后情况将研究组分为预后良好组、预后不良组,对比两组一般资料与血清miR-130a-3p、miR-326、诱导痰EOS水平。经Logistic单因素、多因素分析影响BA患儿预后的危险因素。建立Logistic回归交互作用模型,分析miR-130a-3p、miR-326与EOS交互作用与BA患儿预后的关系。绘制ROC曲线,分析二元Logistics模型预测支气管哮喘患儿预后不良的价值。结果研究组血清miR-130a-3p、miR-326相对表达量比对照组低,诱导痰EOS水平比对照组高(P<0.05);经Pearson相关性显示,血清miR-130a-3p、miR-326相对表达量与诱导痰EOS水平呈负相关(r=-0.532、-0.358,P均<0.001);研究组120例BA患儿中有20例预后不良,占比16.67%;预后不良组血清miR-130a-3p、miR-326相对表达量比预后良好组低,诱导痰EOS水平比预后良好组高(P<0.05);经Logistic回归分析发现,血清miR-130a-3p、miR-326是BA患儿预后的保护因素(OR=0.109、0.162,P=0.004、0.002,95%CI:0.352~0.465、0.270~0.882),诱导痰EOS是危险因素(OR=8.656,P<0.001,95%CI:1.122~131.615);miR-130a-3p、miR-326与EOS产生交互作用,增加BA患儿预后不良发生的风险性(P<0.05);建立二元Logistics模型,绘制ROC曲线发现,miR-130a-3p+EOS预测BA患儿预后不良发生的AUC为0.965(95%CI:0.959~0.992),miR-326+EOS预测BA患儿预后不良发生的AUC为0.924(95%CI:0.885~0.962)。结论血清miR-130a-3p、miR-326水平在BA患�Objective To analyze the relationship between serum micro-Ribonucleic acid(miR)-130a-3p,miR-326 and airway inflammation in children with bronchial asthma(BA)and the interactive effect on prognosis.Methods 120 children with BA who received treatment in our hospital from June 2019 to July 2021 were selected as the study group,and another 120 healthy cases who underwent physical examination in our hospital during the same period were selected for inclusion as the control group.Real-time fluorescence quantitative reverse transcription polymerase chain reaction(qRT-PCR)was used to detect serum miR-130a-3p and miR-326 levels between the two groups,and airway inflammatory factor[induced sputum eosinophil(EOS)]levels were measured,and the relative expression of serum miR-130a-3p and miR-326 was analyzed for the correlation between serum miR-130a-3p,miR-326 relative expression and induced sputum EOS levels by Pearson correlation analysis.During follow-up,the study group was divided into the good prognosis group and the poor prognosis group according to their prognosis,and the general data of the two groups were compared with serum miR-130a-3p,miR-326 and induced sputum EOS levels.The risk factors affecting the prognosis of children with BA were analyzed by Logistic uni-factor and multi-factors analysis.Logistic regression interaction model was established to analyze the relationship between miR-130a-3p,miR-326 and EOS interaction and the prognosis of children with BA.ROC curves were plotted to analyze the value of binary logistic model in predicting poor prognosis in children with bronchial asthma.Results The relative expression of serum miR-130a-3p and miR-326 in the study group was lower than that in the control group,and the level of induced sputum EOS was higher than that in the control group(P<0.05).As shown by Pearson′s correlation,there was a negative correlation between the relative expression of serum miR-130a-3p and miR-326 and the level of induced sputum EOS(r=-0.532,-0.358,all P<0.001).In the study group,20
