慢性阻塞性肺疾病患者血清miR-132、miR-22-3p表达水平及临床意义

摘  要:目的探究微小RNA-132(miR-132)和微小RNA-22-3p(miR-22-3p)在慢性阻塞性肺疾病(简称慢阻肺)患者血清中的表达变化以及其对于慢阻肺发生的临床诊断意义。方法收集2021年3月-2023年12月在本院治疗的140名慢阻肺患者及135名健康体检者的血清,同时收集其性别、年龄等一般资料以及肺功能指标。采用实时荧光定量的方法检测受试者血清中miR-132和miR-22-3p相对表达水平;采用Pearson法分析血清miR-132和miR-22-3p相对水平与患者一般资料以及肺功能指标的相关性;采用Logistic回归分析影响慢阻肺发生的危险因素和保护因素。结果与健康对照组相比,慢阻肺患者血清中miR-132表达水平降低,miR-22-3p表达水平升高(均P<0.05)。急性加重期(Acute exacerbation of COPD,AECOPD)患者miR-132表达水平低于稳定期慢阻肺患者,miR-22-3p表达水平高于稳定期慢阻肺患者(P<0.05)。相关性分析结果显示,慢阻肺患者血清中miR-132表达水平与第一秒用力呼气容积(Forced expiratory volume in one second,FEV 1)和用力肺活量(Forced vital capacity,FVC)的比值(FEV 1/FVC)以及FEV 1占预计值的百分比(FEV 1%predicted,FEV 1%pred)呈正相关(均P<0.05);血清中miR-22-3p表达水平与FEV 1/FVC、FEV 1%pred呈负相关(均P<0.05)。Logistic回归分析显示FEV 1/FVC、FEV 1%pred以及血清中miR-132是慢阻肺的保护因素,吸烟史以及血清中miR-22-3p是慢阻肺的危险因素(均P<0.05)。结论慢阻肺患者血清中miR-132表达水平降低,miR-22-3p表达水平升高,二者与慢阻肺患者的病情及肺功能指标明显相关,可能作为评估慢阻肺患者病情的重要指标。Objective To investigate the expression changes of miR-132 and miR-22-3p in the serum of patients with chronic obstructive pulmonary disease(COPD)and their clinical diagnostic significance for COPD.Methods Serum samples were collected from 140 COPD patients and 135 healthy individuals who were treated in our hospital from March 2021 to December 2023.Their general information such as gender,age,and lung function indicators were collected.Real-time fluorescence quantification method was applied to detect the relative expression levels of miR-132 and miR-22-3p in the serum of subjects.Pearson method was applied to analyze the correlation between the relative levels of serum miR-132 and miR-22-3p with patient general information and lung function indicators.Logistic regression was applied to analyze the risk and protective factors that affected the occurrence of COPD.ROC curve was applied to evaluate the clinical significance of miR-132,miR-22-3p,and their combined detection in the diagnosis of COPD.Results Compared with the healthy control group,the expression level of miR-132 in the serum of COPD patients decreased,while the expression level of miR-22-3p increased(both P<0.05).The expression level of miR-132 in patients with acute exacerbation stage(AECOPD)was lower than that in patients with stable COPD,and the expression level of miR-22-3p was higher than that in patients with stable COPD(P<0.05).The correlation analysis showed that the expression level of miR-132 in serum of COPD patients was positively correlated with the ratio of forced expiratory volume in one second to forced vital capacity(FEV 1/FVC)and FEV 1 as a percentage of the expected value(FEV 1%pred)(both P<0.05).The expression level of miR-22-3p in serum was negatively correlated with FEV 1/FVC and FEV 1%pred(both P<0.05).Logistic regression analysis showed that FEV 1/FVC,FEV 1%pred,and miR-132 in serum were protective factors for COPD,while smoking history and miR-22-3p in serum were risk factors for COPD(all P<0.05).Conclusion The expression leve
