基于数据挖掘与网络药理学分析于世家教授治疗糖尿病肾病的用药规律及作用机制

摘　　要：目的:通过中医传承计算平台V3.0软件挖掘于世家教授治疗糖尿病肾病(Diabetic Kidney Disease,DKD)的用药规律,获取最佳药对,利用网络药理学研究其作用机制。方法:收集整理2019年7月—2021年6月就诊于辽宁中医药大学附属医院由于世家教授亲自接诊的DKD患者病例信息;组建处方数据库;通过中医传承计算平台V3.0分析总结最佳药物配伍并进行网络药理学分析。结果:研究共筛选出有效处方101首,使用中药65味。“黄芪–丹参”为最佳配伍,该药对共含有有效成分76个,靶点244个,主要活性成分为槲皮素、山柰酚、木犀草素,可能通过糖尿病并发症中的晚期糖基化终末产物与其受体(Advanced Glycation End product–Receptor for Advanced Glycation End Products,AGE–RAGE)信号通路、流体剪切应力与动脉粥样硬化、白细胞介素(Interleukin,IL)–17信号通路、肿瘤坏死因子(Tumor Necrosis Factor,TNF)信号通路等发挥治疗作用。结论:于世家教授认为瘀血阻络贯穿本病始终,根据“久病必瘀”的思想,将益气活血贯穿治疗的始终。Objective:To mine the medication rules of Prof.YU Shijia’s treatment of diabetic kidney disease(DKD)through the software of Traditional Chinese Medicine Inheritance Computing Platform V3.0,to obtain the optimal couplet medicinals,and to study the preliminary mechanism of its action by using network pharmacology.Methods:We collected and sorted out the case information of DKD patients treated by Prof.YU Shijia in the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine from July 2019 to June 2021.The prescription database was established.Through the Chinese Medicine Heritage Computing Platform V3.0,the optimal couplet medicinals was summarized,and the network pharmacology analysis was carried out.Results:A total of 101 effective prescriptions were screened in this study,65 flavors of traditional Chinese medicines were used in the prescriptions.Huangqi(Radix Astragali)-Danshen(Radix et Rhizoma Salviae Miltiorrhizae)was the optimal couplet medicinals,which contained a total of 76 active ingredients and 244 targets,with quercetin,kaempferol and luteolin as the main active ingredients.Conclusion:Prof.YU Shijia believes that blood stasis and obstruction of collaterals are the main causes of this disease.Based on the idea of“long term diseases must have blood stasis”,the couplet medicinals Huangqi-Danshen is commonly used to promote Qi and blood circulation throughout the treatment.This couplet medicinals may play a therapeutic role in the complications of diabetes through AGE-RAGE signaling pathway,fluid shear stress and atherosclerosis,IL-17 signaling pathway,TNF signaling pathway and other pathways.
