基于黄元御六经气化理论探讨燥痹证治

摘　　要：干燥综合征是一种临床常见的免疫系统疾病,中医称之为“燥痹”。随着干燥综合征发病率的提高和对其病理认识不断完善,此病越来越受到临床研究的重视。中医中药在人体免疫调节方面具有独特的优势。《伤寒论》将人体划为六经统属,正气不足则三阴致病,病机与免疫低下所致各种免疫系统疾病相合,在诊断和治疗方面另辟蹊径,此后医家对此多有发展。黄元御“六经气化”理论依托经典,创新发挥,深挖人体气机变化,指出“少阴亏虚、相火上逆”是燥痹发病的机理。从六经气化角度出发,黄氏认为肾水亏于下,相火郁于上,己土湿陷,升降失常,故津液不生而致孔窍干燥。肾水为人体津液生成和输布的关键环节,津液的生成和运输都需要阳气的支撑。相火上郁,不能下温肾水,脾胃运化功能失常,津液无法生成,人体四肢百骸不能得到濡养,则表现为口干眼干等各种症状。黄元御以六经气化理论探讨“伏少阳、潜雷火”的治疗方针,一方面提出火邪宜透达,另一方面引火归元,使人体阳气恢复正常气化功能,恢复脾胃升降,使三焦通路畅达,水液输布无碍,人体得以康健。研究基于黄元御六经气化理论探讨燥痹证治疗,以期为临床扩展思路。Sjogren’s syndrome is a common clinical immune system disease,called“Zao Bi”(燥痹)in traditional Chinese medicine.With the increase of incidence and the continuous improvement of pathological understanding,more and more attention has been paid to clinical research.In terms of human immune regulation,Chinese medicine has unique advantages.According to Shanghan Lun(《伤寒论》),the human body is divided into six meridians.Deficiency of healthy Qi(气)will cause three Yin(阴)diseases.The pathogenesis often coincides with the low level of immunity.Doctors of past dynasties have further developed this theory.HUANG Yuanyu’s“six-meridian Qi transformation theory”is innovative on the basis of classics and explores the changes of human body’s Qi mechanism.He pointed out that“deficiency of Shaoyin(少阴)and inversion of ministerial fire”are the pathogenesis of“Zao Bi”.From the perspective of six-meridian Qi transformation theory,he believed that the lower energizer kidney water is insufficient,the upper energizer ministerial fire is stagnant,the earth is collapsing,and the Qi exiting and entering are abnormal,so the body fluid is not produced,which led to dryness impediment syndrome.For the treatment of“Zao Bi”,HUANG Yuanyu put forward the treatment policy of“subdue Shaoyang(少阳)and hidden Leihuo(雷火)”based on the theory of six-meridian Qi transformation.On the one hand,he proposed to recover the ascending and descending functions of fire.On the other hand,he proposed to induce ministerial fire to restore the kidney,so that the Yang of the human body can be transformed normally.At the same time,he suggested that we should pay attention to regulate the spleen and stomach,so that the triple energizer pathway will be smooth,and the water and liquid can be transported without obstruction,so that the human body can be healthy.This paper discusses the treatment of Sjogren’s syndrome based on HUANG Yuanyu’s theory of six-meridian Qi transformation,to expand the clinical thinking.
