何泽教授助阳通络法治疗肥胖2型糖尿病经验浅析

摘　　要：糖尿病是一种以血糖浓度升高为特征的慢性疾病。肥胖是指由于能量代谢失衡导致体内脂肪过度蓄积的慢性代谢性疾病。研究显示,约2/3的2型糖尿病患者合并超重或肥胖。肥胖2型糖尿病是一种与内外环境、遗传因素、胰岛素抵抗等多种因素相关的代谢性疾病,发病率呈逐年上升趋势。此病显著增加了心脏病、呼吸暂停综合征、癌症等病的患病风险,严重威胁患者的身心健康。中医认为,肥胖2型糖尿病属“脾瘅”“消渴”等范畴,该病缠绵难愈,本虚标实为其主要特点。中医治疗肥胖2型糖尿病具有显著优势,既可调节糖脂代谢水平,改善胰岛素抵抗,又可减轻体质量。何泽教授从事中医临床工作近30年,对肥胖2型糖尿病病机和治法的研究颇丰。他基于“阳化气、阴成形”“血不利则为水”理论,认为“阳气不足,脾失健运,痰瘀阻络,水湿内停”是肥胖2型糖尿病发病的核心病机,并提出以助阳通络为治疗大法,具体是在生活方式干预的基础上,宣阳气,祛浊邪,将助阳通络法贯穿始终。研究结合案例,介绍何教授助阳通络治疗肥胖2型糖尿病经验,以期为临床提供参考。iabetes is a chronic disease characterized by elevated blood glucose concentrations.Obesity is a chronic metabolic disease characterized by excessive accumulation of body fat due to imbalance in energy metabolism.Studies have shown that about 2/3 of type 2 diabetes is combined with overweight or obesity.As a metabolic disease,the incidence of obesity type 2 diabetes is increasing year by year,significantly increasing the risk of heart disease,apnea syndrome,cancer and other diseases,seriously threatening the physical and mental health of patients.Chinese medicine believes that obesity type 2 diabetes belongs to the category of Pidan(脾瘅),Xiaoke(消渴),etc.The disease has a long course,lingering and difficult to cure,and its main feature is the deficiency in origin and excess in superficiality.Chinese medicine has significant advantages in treating obesity type 2 diabetes,which can regulate glucose and lipid metabolism levels,improve insulin resistance,and reduce body weight,among other things.This article introduces Prof.HE Ze’s experience in treating obesity type 2 diabetes by using the method of assist Yang(阳)and unblock meridians.Prof.HE Ze has been engaged in clinical work of traditional Chinese medicine for nearly 30 years,and has done a lot of researches on the pathogenesis and treatment of obesity type 2 diabetes.Based on the theory of Yang transforming Qi(气)while Yin(阴)constituting form and“if blood flow is not smooth,it will lead to edema”,he believes that“Yang deficiency,spleen dysfunction,phlegm and blood stasis blocking collaterals,and internal stagnation of water and dampness”are the core pathogenesis of obesity type 2 diabetes,and proposes to Assist Yang and unblock meridians as the treatment of obesity type 2 diabetes.In clinical practice,Prof.HE believes that on the basis of lifestyle intervention,the method of invigorating Yang Qi and removing turbid pathogens should be used throughout.This paper introduces Prof.HE’s experience in the treatment of obese type 2 diabetes by As
