循经督刺结合艾灸治疗脾肾阳虚型骨质疏松性椎体压缩性骨折临床研究

摘　　要：目的:观察循经督刺法结合艾灸治疗脾肾阳虚型骨质疏松性椎体压缩性骨折(Osteoporotic Vertebral Compression Fracture,OVF)的临床疗效。方法:将96例脾肾阳虚型OVCF患者根据随机数字表法分为常规组和督刺组,每组48例。两组均口服基础药物塞来昔布胶囊、阿法骨化醇软胶囊和钙尔奇片并联合艾灸、针刺穴位,常规组选取常规穴位针刺,督刺组采用循经督刺法结合艾灸。治疗1个月后对比两组的临床疗效,观察治疗前后两组患者的骨密度变化、椎体Oswestry功能障碍指数(Oswestry Disability Index,ODI)评分、伤椎前缘丢失高度及Cobb角变化。结果:督刺组的总有效率显著高于常规组,差异有统计学意义(P<0.05)。两组治疗1个月后临床症状和观察指标均较治疗前有明显好转,差异有统计学意义(P<0.05),其中督刺组治疗1个月后伤椎ODI评分明显低于常规组,骨密度明显高于常规组,伤椎前缘丢失高度及Cobb角明显低于常规组,差异有统计学意义(P<0.05)。结论:循经督刺结合艾灸能提高脾肾阳虚型OVCF患者的治疗效果,有效缓解OVCF患者疼痛,改善伤椎椎体功能,促进局部骨折愈合,为OVCF的治疗提供一定的思路。Objective:To observe the clinical efficacy of acupuncture at acupoints on Du vessel(督脉)plus moxibustion in the treatment of osteoporotic vertebral compression fracture of the spleen-kidney Yang(阳)deficiency type.Methods:Ninetysix patients with OVCF of the spleen-kidney Yang deficiency type were divided into the routine acupuncture group and the governor needling group by the random number table method,with 48 cases in each group.Both groups were treated with basic oral drugs celecoxib capsules,alfacalcidol soft capsules and caltrate tablets,and moxibustion and acupuncture was used in combination.Additionally,the routine acupuncture group was treated by conventional acupuncture,while the governor needling group was treated by acupuncture at acupoints on Du vessel plus moxibustion.After 1 month of treatment,the clinical effects,bone mineral density changes,ODI scores,injured anterior vertebral lose height and Cobb angle changes were compared between the two groups before and after treatment.Results:The total efficiency in the governor needling group was significantly higher than that in the routine acupuncture group(P<0.05).After 1 month of treatment,the clinical symptoms and observation indexes in the groups were significantly ameliorated compared with those before treatment,and the differences were statistically significant(P<0.05).The ODI score of injured vertebrae in the governor needling group was significantly lower than that in the routine acupuncture group,the bone mineral density was significantly higher than that in the routine acupuncture group,and the injured anterior vertebral lose height and Cobb angle were significantly lower than those in the routine acupuncture group;the differences were statistically significant(P<0.05).Conclusions:Acupuncture at acupoints on Du vessel plus moxibustion can improve the therapeutic effect of OVCF patients with spleen-kidney Yang deficiency,effectively relieve pain,improve the function of injured vertebrae,and promote local fracture healing,providing a certain
