调任通督针刺法治疗早发性卵巢功能不全的机制研究进展

摘　　要：早发性卵巢功能不全(Premature Ovarian Insufficiency,POI)是女性卵巢功能减退的一种疾病,主要表现为月经紊乱、雌激素水平下降、促性腺激素水平升高、不孕等。西医学对POI的发病机理尚无定论,多认为与遗传、环境、免疫等多种因素导致的卵巢功能受损有关。目前西医仍无有效的方法恢复卵巢功能,主要治疗手段为激素替代疗法,通过人工补充雌激素、孕激素来改善症状。在中医学中,POI属于“经水早断”“闭经”“不孕”等范畴,主要病机为肾虚,与肝、脾密切相关,并且受奇经八脉中任脉、督脉、冲脉、带脉共同影响,受肾–天癸–冲任–胞宫轴的调控,主要采用中药、针刺、艾灸、拔罐等中医特色疗法治疗。近年来,POI患者呈现出数量逐年上升、年轻化的态势,对女性的生育率及身心健康造成极大威胁,因而寻求一种绿色有效、不良反应小并适用于普遍人群的干预方法显得尤为重要。随着对POI的认识越来越深入,采用中医针灸疗法也成为一种新的治疗趋势。本研究整合近些年采用调任通督针刺法治疗POI的相关研究,得出该针刺疗法可通过补益肾气、调和脏腑阴阳气血,在不同程度上改善POI患者的临床症状和激素水平,以促进卵巢功能的自我修复及增强机体的免疫功能,为POI的临床改良治疗提供方向和建议,该疗法具有一定的推广使用价值。remature ovarian insufficiency(POI)is a disease of female decreased ovarian function,primarily manifested as menstrual disorder,estrogen level decline,gonadotropin level increase,infertility and so on.The pathogenesis of POI is still inconclusive in Western medicine,and it is believed to be related to ovarian function impairment caused by a variety of factors such as genetics,environment,and immunity.Currently,Western medicine still has no effective method to restore ovarian function,and the main treatment means is hormone replacement therapy,which ameliorates clinical symptoms by artificially supplementing estrogen and progesterone.In traditional Chinese medicine,POI belongs to the categories of“early cessation of menstruation”,“amenorrhea”,“infertility”,etc.It is believed that the main cause of POI lies in kidney deficiency,closely related to liver and spleen,is influenced jointly by extraordinary vessels such as Ren vessel(任脉),Du vessel(督脉),Chong vessel(冲脉),Dai vessel(带脉),and is regulated by the kidney-menstruation-Chong vessel-uterus axis.In terms of the treatment,Chinese medicines,acupuncture,moxibustion,cupping,and other distinctive therapies of traditional Chinese medicine are used clinically.In recent years,the number of POI patients has been increasing year by year and becoming younger,posing a great threat to women’s fertility and physical and mental health.Therefore,currently seeking a green,effective,fewer adverse reactions,and widely applicable intervention method is particularly important.With the deepening of our understanding on POI,the application of traditional Chinese acupuncture and moxibustion therapy has also become a new popular trend.In this article,the relevant studies on treating POI by acupuncture for regulating Ren vessel and unchoking Du vessel in recent years are summarized,and it is concluded that this acupuncture therapy can relieve the clinical symptoms and hormone levels of POI patients to varying degrees by the methods of tonifying kidney Qi(气),an
