摘 要:目的:探讨基于头颈部CT血管造影(CTA)检查人群的原始图像筛查偶发甲状腺结节的发生概率及其结节的特点。方法:选取于安徽医科大学附属阜阳医院2021年7月—2023年12月就诊并完善头颈部CTA检查的患者674例。对其资料行回顾性分析。比较结节组和正常组的一般资料,分析偶发甲状腺结节的影响因素和影像学特征。结果:结节组和正常组的性别、年龄比较,差异有统计学意义(P<0.05);二元Logistic回归分析发现年龄的增加、女性是偶发甲状腺结节的独立危险因素(P<0.05),且女性患病风险是男性2.381倍,年龄每增加1岁其患病风险增加2.3%;不同性别的偶发甲状腺结节在位置、数量、最大径、纵横比、边界、密度上比较,差异无统计学意义(P>0.05);偶发甲状腺结节不同位置、数量、最大径、纵横比、边界、密度的年龄比较,差异无统计学意义(P>0.05)。结论:头颈部CTA检查的患者中甲状腺偶发结节的患病率较高,女性及年龄的增长是偶发甲状腺结节的独立危险因素。Objective To investigate the incidence and characteristics of incidental thyroid nodules(ITNs)identified through screening raw images of patients undergoing head and neck CT angiography(CTA).Methods A retrospective analysis was conducted on 674 patients who underwent head and neck CTA at Fuyang Hospital of Anhui Medical University from July 2021 to December 2023.The general data of the nodule group and the normal group were compared,and the influencing factors and imaging features of incidental thyroid nodules were analyzed.Results There were significant differences in gender and age between the nodule group and the normal group(P<0.05).Binary logistic regression analysis revealed that increasing age and female gender were independent risk factors for ITNs(P<0.05).Females had a 2.381-fold higher risk than males,and the risk increased by 2.3%for each additional year of age.There were no statistically significant differences in the location,number,maximum diameter,aspect ratio,boundary,or density of ITNs between different genders(P>0.05).Similarly,there were no statistically significant differences in age among ITNs with different locations,numbers,maximum diameters,aspect ratios,boundaries,or densities(P>0.05).Conclusion The incidence of ITNs is relatively high in patients undergoing head and neck CTA,and female gender and increasing age are independent risk factors for ITNs.
