摘  要:目的:探讨动态CT增强在原发性周围型小细胞肺癌与肺鳞癌中的鉴别诊断价值。方法:选取2019年7月—2024年7月泗阳医院收治的60例肺癌患者,均行动态CT增强检查及病理学检查,按病理类型分为小细胞肺癌组20例,肺鳞癌组40例,观察患者动态CT增强特征与临床病理类型的关系。结果:小细胞肺癌组与肺鳞癌组病灶发病部位比较,差异无统计学意义(P>0.05);两组动态CT增强动脉期(30 s)、静脉期(70 s)的强化程度、强化净增值、强化率比较,差异有统计学意义(P<0.05);两组病灶的强化形式、形状、毛刺、空洞、纵隔肺门淋巴结肿大、淋巴结融合等影像特征比较,差异有统计学意义(P<0.05);两组病灶的分叶征、棘突、胸膜凹陷征、钙化、胸膜转移、心包积液等影像特征比较,差异无统计学意义(P>0.05)。结论:动态CT增强在原发性周围型小细胞肺癌与肺鳞癌鉴别诊断中具有较高的价值,临床可根据病灶影像特征对病理分型进行判断。Objective To explore the diagnostic value of dynamic CT enhancement in differentiating primary peripheral small cell lung cancer from squamous cell carcinoma of the lung.Methods A total of 60 lung cancer patients admitted to Siyang Hospital from July 2019 to July 2024 were selected.All patients underwent dynamic CT enhancement and pathological examinations.Based on pathological types,they were divided into a small cell lung cancer group(20 cases)and a squamous cell carcinoma group(40 cases).The relationship between dynamic CT enhancement characteristics and clinicopathological types was analyzed.Results There was no statistically significant difference in the lesion locations between the small cell lung cancer group and the squamous cell carcinoma group(P>0.05).However,significant differences were observed in the degree of enhancement,net enhancement value,and enhancement rate during the arterial phase(30 s)and venous phase(70 s)of dynamic CT enhancement(P<0.05).Additionally,significant differences were found in imaging features such as enhancement patterns,shape,spiculation,cavitation,mediastinal and hilar lymph node enlargement,and lymph node fusion(P<0.05).No significant differences were observed in imaging features such as lobulation,spiculation,pleural indentation,calcification,pleural metastasis,and pericardial effusion(P>0.05).Conclusion Dynamic CT enhancement has high diagnostic value in differentiating primary peripheral small cell lung cancer from squamous cell carcinoma of the lung.Clinical pathological typing can be determined based on the imaging characteristics of the lesions.
