退行性心脏瓣膜疾病患者的超声心动图诊断及准确率评价

摘　　要：目的:探讨超声心动图对退行性心脏瓣膜疾病(DVHD)的诊断效能及诊断准确率。方法:选取2023年10月—2024年6月睢宁县中医院收治的110例疑似DVHD患者为研究对象,以病理检查为金标准,确诊为DVHD患者纳入阳性组,未确诊为DVHD患者纳入阴性组。比较两组心功能指标及超声血流指标,并绘制受试者工作特征曲线分析超声心动图对DVHD的诊断效能。结果:病理结果检出阳性43例,阴性67例;超声心动图检出阳性41例,阴性69例,准确率为96.36%;阳性组左心室收缩末期容积、左心室舒张末期容积高于阴性组,差异有统计学意义(P<0.05);阳性组二尖瓣舒张晚期血流速度高于阴性组,二尖瓣舒张早期血流速度低于阴性组,差异有统计学意义(P<0.05);超声心动图参数联合诊断DVHD的曲线下面积高于单一诊断。结论:超声心动图对DVHD的诊断效能较好,准确率较高,值得临床应用。Objective To explore the diagnostic efficacy and diagnostic accuracy of echocardiography for degenerative valvular heart disease(DVHD).Methods A total of 110 patients with suspected DVHD admitted to Suining TCM Hospital from October 2023 to June 2024 were selected as the study subjects.Using pathological examination as the gold standard,patients diagnosed with DVHD were included in the positive group,while those not diagnosed with DVHD were included in the negative group.Cardiac function indicators and ultrasound hemodynamic parameters were compared between the two groups.Receiver operating characteristic curves were plotted to analyze the diagnostic efficacy of echocardiography for DVHD.Results Pathological examination showed 43 positive cases and 67 negative cases.Echocardiography detected 41 positive cases and 69 negative cases,with an accuracy rate of 96.36%.The left ventricular end-systolic volume and left ventricular end-diastolic volume in the positive group were higher than those in the negative group,with statistically significant differences(P<0.05).The A wave in the positive group was higher than that in the negative group,while the E wave was lower than that in the negative group,with statistically significant differences(P<0.05).The area under the curve of the combined echocardiographic parameters for the diagnosis of DVHD was higher than that of a single parameter diagnosis.Conclusion Echocardiography demonstrates good diagnostic efficacy and high accuracy for DVHD,making it worthy of clinical application.
