摘 要:目的:探讨临床联合高频超声影像对乳腺癌新辅助治疗疗效的预测价值。方法:选取2018年1月—2023年11月在惠州市中心人民医院进行乳腺癌新辅助治疗(NAT)并手术的患者120例,根据NAT疗效评价结果将患者分为病理完全缓解(PCR)组和非病理完全缓解(nPCR)组,采用单因素及多因素Logistic回归方法分析乳腺癌患者NAT后达到PCR的独立预测指标,并绘制受试者工作特征(ROC)曲线。结果:多因素Logistic回归分析结果显示:乳腺癌肿块的超声特征边缘无毛刺、后方回声无衰减及免疫组化中ER≤10%是乳腺癌新辅助治疗后达到PCR的独立预测因子,回归模型ROC曲线的曲线下面积为0.852。结论:乳腺癌肿块边缘无毛刺、后方回声无衰减的超声成像特征及ER低表达可作为乳腺癌NAT后PCR的独立预测因子,NAT前高频超声检查及ER表达情况可为乳腺癌新辅助治疗疗效评估提供重要参考依据。Objective To explore the predictive value of clinical combined high-frequency ultrasound imaging on the efficacy of neoadjuvant therapy for breast cancer.Methods A total of 120 patients who underwent neoadjuvant therapy(NAT)and surgery for breast cancer in Huizhou Central People's Hospital from January 2018 to November 2023 were retrospectively enrolled.According to the NAT efficacy evaluation results,patients were divided into pathologic complete response(PCR)group and non-pathologic complete response(nPCR)group.Univariate and multivariate Logistic regression methods were used to analyze the independent predictive indicators for breast cancer patients to achieve PCR after NAT,and the receiver operating characteristic(ROC)curve was plotted.Results The multivariate Logistic regression analysis results indicated that the ultrasound characteristics of breast cancer mass with no burr at the edge,no attenuation of posterior echo and ER≤10%in immunohistochemistry were the independent predictive factors for achieving PCR after breast cancer NAT,and the area under the curve of the ROC curve of the regression model was 0.852.Conclusion The ultrasound imaging characteristics of breast cancer mass with no burr at the edge,no attenuation of posterior echo and low expression of ER can serve as independent predictors of PCR after NAT in breast cancer,and high-frequency ultrasound examination and ER expression before NAT can provide an important reference for the efficacy assessment of neoadjuvant therapy in breast cancer.
