GLP-1RA辅助治疗2型糖尿病并发周围神经病变患者的效果及对内脏脂肪面积影响

摘  要:目的:研究胰高血糖素样肽-1受体激动剂(GLP-1RA)在2型糖尿病并发周围神经病变患者中的应用效果及对内脏脂肪面积的影响。方法:研究方法为回顾性分析,纳入2023年3月-2024年3月兴义市人民医院内分泌代谢病科收治的100例2型糖尿病并发周围神经病变患者,根据治疗方式不同分为研究组(n=50)与对照组(n=50)。研究组联用司美格鲁肽注射液、盐酸二甲双胍、甲钴胺治疗,对照组用盐酸二甲双胍联合甲钴胺治疗。比较两组血糖控制效果、神经传导速度、血脂水平、内脏及皮下脂肪面积。结果:治疗后两组糖化血红蛋白(HbA1c)、空腹血糖(FBG)、餐后2 h血糖(2 h PBG)、胰岛素抵抗指数(HOMA-IR)均下降,且研究组均低于对照组,差异均有统计学意义(P<0.05);治疗后两组正中神经与腓总神经的运动神经传导速度(MNCV)、感觉神经传导速度(SNCV)均升高,且研究组均高于对照组,差异均有统计学意义(P<0.05);治疗后两组腹部内脏脂肪面积(VFA)、皮下脂肪面积(SFA)均降低,且研究组均低于对照组,差异均有统计学意义(P<0.05);治疗后研究组甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白(LDL)均低于对照组,高密度脂蛋白(HDL)高于对照组,差异均有统计学意义(P<0.05)。结论:司美格鲁肽可明显改善2型糖尿病并发周围神经病变患者血糖、血脂,提高神经传导速度,减少内脏脂肪面积。Objective:To study the application effect of glucagon-like peptide-1 receptor agonists(GLP-1RA)in patients with type 2 diabetes mellitus complicated with peripheral neuropathy and its influence on visceral fat area.Method:The study method was retrospective analysis.100 patients with type 2 diabetes mellitus complicated with peripheral neuropathy admitted to the Department of Endocrine Metabolism Disease,Xingyi People's Hospital from March 2023 to March 2024 were included,and they were divided into study group(n=50)and control group(n=50)according to different treatment methods.The study group was treated with Semaglutide Injection and Metformin Hydrochloride and Mecobalamin,and the control group was treated with Metformin Hydrochloride and Mecobalamin.Blood glucose control effect,nerve conduction velocity,blood lipid levels,visceral and subcutaneous fat area were compared between the two groups.Result:After treatment,glycosylated hemoglobin(HbA1c),fasting blood glucose(FBG),2 h postprandial blood glucose(2 h PBG)and homeostasis model assessment of insulin resistance(HOMA-IR)in both groups were decreased,and those in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the motor nerve conduction velocity(MNCV)and sensory nerve conduction velocity(SNCV)of median nerve and common peroneal nerve were increased in both groups,and those in the study group were higher than those in the control group,the differences were statistically significant(P<0.05).After treatment,the abdominal visceral fat area(VFA)and subcutaneous fat area(SFA)of both groups were decreased,and those in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,triglyceride(TG),total cholesterol(TC)and low density lipoprotein(LDL)in the study group were lower than those in the control group,and high density lipoprotein(HDL)was higher than that in the control group,the differences were statistically sig
