摘　　要：目的:探讨桃红四物汤对老年腰背部软组织损伤合并骨质疏松性椎体压缩性骨折(osteoporotic vertebral compression fractures,OVCF)患者经皮椎体成形术(percutaneous vertebroplasty,PVP)术后恢复情况的影响。方法:前瞻性选取2021年1月—2022年7月于江西中医药大学第二附属医院收治的100例老年腰背部软组织损伤合并OVCF患者作为研究对象,按随机数字表法分为联合组(n=50)和对照组(n=50)。两组患者均行PVP,联合组额外联合桃红四物汤治疗。比较两组患者术后2个月的临床疗效、中医症候积分、骨转换标志物水平、视觉模拟评分法(visual analogue scale,VAS)评分、Oswestry功能障碍指数(Oswestry disability index,ODI)。结果:联合组总有效率显著高于对照组(P<0.05);术后2个月,联合组的中医症候各项积分均低于对照组(P<0.05);术后2个月,两组的骨转换标志物均较术前明显改善,且联合组的骨特异性碱性磷酸酶(BALP)和骨钙素(BGP)水平均高于对照组,Ⅰ型胶原羧基端肽β特殊序列(β-CTX)和Ⅰ型前胶原羧基端前肽(PICP)水平均低于对照组(P<0.05);术后2个月,联合组的VAS和ODI评分均低于对照组(P<0.05)。结论:桃红四物汤对老年腰背部软组织损伤合并OVCF患者PVP术后的临床疗效显著,能改善中医症候和骨转换标志物水平,减轻腰背部疼痛,促进椎体功能恢复,是提升此类患者术后生活质量的有效方法。Objective:To investigate the effect of Taohong Siwu Decoction on the recovery after percutaneous vertebroplasty(PVP)in elderly patients with soft tissue injuries of the low back combined with osteoporotic vertebral compression fractures(OVCF).Method:A total of 100 elderly patients with lower back soft tissue injury combined with OVCF admitted to the Second Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine from January 2021 to July 2022 were prospectively selected as the research subjects and divided into a combined group(n=50)and a control group(n=50)by random number table method.Both groups of patients underwent PVP,and the combined group was additionally treated with Taohong Siwu Decoction.The clinical efficacy,traditional Chinese medicine(TCM)symptom scores,levels of bone turnover markers,visual analogue scale(VAS)scores,and Oswestry disability index(ODI)of the two groups of patients were compared 2 months after surgery.Result:The total effective rate of combined group was significantly higher than that of control group(P<0.05).2 months after surgery,the scores of TCM symptoms in the combined group were lower than those in the control group(P<0.05).2 months after surgery,bone turnover markers in both groups were significantly improved compared with those before surgery,and bone alkaline phosphatase(BALP)and bone gla-protein(BGP)levels in the combined group were higher than those in the control group,β-carboxyⅠterminal peptide(β-CTX)and propeptide of typeⅠprocollagen(PICP)levels were lower than those in the control group(P<0.05).2 months after surgery,VAS and ODI scores of the combined group were lower than those of the control group(P<0.05).Conclusion:Taohong Siwu Decoction has a significant clinical effect on elderly patients with lower back soft tissue injury combined with OVCF after PVP.It can improve TCM symptoms and bone turnover marker levels,relieve lumbar and back pain,promote vertebral body function recovery,and is an effective method to improve the postoperative quality
