摘　　要：目的:分析比较老年肾结石(KS)患者采取输尿管软镜碎石术(FURL)与经皮肾镜碎石术(PCNL)的效果。方法:选择2022年2月—2024年4月福建中医药大学附属第三人民医院收治的80例老年KS患者,随机分对照组、观察组,各40例。对照组行PCNL治疗,观察组行FURL治疗。比较两组的手术指标、肾功能指标、炎症因子水平、结石清除率及术后并发症发生率。结果:观察组术中出血量少于对照组,手术时间、住院时间、术后卧床时间均短于对照组,差异均有统计学意义(P<0.05)。治疗后,两组肾功能、炎症因子指标均高于治疗前,但观察组均低于对照组,差异均有统计学意义(P<0.05)。两组结石清除率及术后并发症发生率比较,差异均无统计学意义(P>0.05)。结论:对于结石直径≤3 cm的老年KS患者,PCNL与FURL结石清除率相近,但FURL对肾功能损伤更小,出血量及炎症反应更少,术后恢复更快,安全性好。Objective:To analyze and compare the effect of flexible ureteroscopic lithotripsy(FURL)and percutaneous nephrolithotomy(PCNL)in elderly patients with kidney stones(KS).Method:A total of 80 elderly KS patients admitted to the Third People's Hospital Affiliated to Fujian University of Traditional Chinese Medicine from February 2022 to April 2024 were randomly divided into control group and observation group,with 40 cases in each group.The control group received PCNL treatment,and the observation group received FURL treatment.The operative indexes,renal function,inflammatory factors,stone clearance rate and postoperative complication rate between the two groups were compared.Result:The intraoperative blood loss in the observation group was less than that in the control group,and the operation time,hospital stay and postoperative bed time were all shorter than those in the control group,the differences were statistically significant(P<0.05).After treatment,renal function and inflammatory factor indexes in two groups were higher than those before treatment,but those in observation group were lower than those in control group,the differences were statistically significant(P<0.05).There were no significant differences in stone clearance rate and postoperative complication rate between the two groups(P>0.05).Conclusion:For elderly KS patients with stone diameter≤3 cm,the stone clearance rate of PCNL and FURL is similar,but FURL has less damage to renal function,less blood loss and inflammation,faster postoperative recovery and good safety.
