腕踝针在火毒蕴结型肛周脓肿术后尿潴留护理中的应用

摘　　要：目的:探讨基于三阴三阳理论的腕踝针在火毒蕴结型肛周脓肿术后尿潴留临床护理中的应用效果。方法:选取2024年1—8月在安徽中医药大学第一附属医院肛肠科诊治的60例火毒蕴结型肛周脓肿术后尿潴留患者,按照随机数字表法分两组,经剔除后,观察组29例和对照组26例。对照组接受常规护理,观察组在对照组基础上添加基于三阴三阳理论的腕踝针干预。比较分析两组治疗后的临床效果、首次排尿通畅情况、首次排尿时间、首次排尿量、首次排尿后残尿量、2 h下腹胀痛评分和生活质量。结果:观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。观察组首次排尿时间早于对照组,首次排尿后膀胱残余尿量低于对照组,首次排尿量多于对照组,首次排尿通畅情况得分、2 h下腹胀痛评分均低于对照组,差异均有统计学意义(P<0.05)。干预后,两组生活质量评分均高于干预前,且观察组均高于对照组,差异均有统计学意义(P<0.05)。结论:基于三阴三阳理论,为火毒蕴结型肛周脓肿术后发生尿潴留患者实施腕踝针干预,可使患者术后尿潴留情况得到改善,有一定的临床应用价值。Objective:To investigate the clinical nursing effect of wrist and ankle acupuncture based on the theory of three Yin and three Yang in the postoperative urinary retention of perianal abscess with fire poison accumulation type.Method:A total of 60 patients with postoperative urinary retention of perianal abscess with fire poison accumulation type in the Proctology Department of the First Affiliated Hospital of Anhui University of Traditional Chinese Medicine from January to August 2024 were selected and divided into two groups according to random number table method.After elimination,29 patients were in the observation group and 26 patients in the control group.The control group received routine nursing,and the observation group received wrist and ankle acupuncture based on the theory of three Yin and three Yang on the basis of the control group.The clinical effect,patency of the first urination,the first urination time,the first urination volume,the residual urine volume after the first urination,the score of abdominal distension and pain 2 h and the quality of life of the two groups were compared and analyzed.Result:The total effective rate of the observation group was higher than that of the control group,the difference was statistically significant(P<0.05).The first urination time of the observation group was earlier than that of the control group,the bladder residual urine volume after the first urination was lower than that of the control group,the first urination volume was more than that of the control group,and the score of the first urination patency and the score of lower abdominal pain at 2 h were lower than those of the control group,the differences were statistically significant(P<0.05).After intervention,the quality of life scores of both groups were higher than those before intervention,and that in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Based on the theory of three Yin and three Yang,wrist and ankle acupuncture
