中医推拿整复手法结合聚焦式冲击波治疗腰肌劳损的临床研究

摘　　要：目的:探讨中医推拿整复手法联合聚焦式冲击波疗法在腰肌劳损治疗中的应用价值。方法:选取福建中医药大学附属三明市中西医结合医院2023年12月—2024年5月收治的腰肌劳损患者,共纳入80例。经随机数字表法分组,对照组与观察组每组40例。对照组采取聚焦式冲击波治疗,观察组采取聚焦式冲击波结合中医推拿整复手法治疗,对比两组临床疗效、疼痛改善情况、功能障碍程度改善情况、腹背肌耐力改善情况。结果:两组治疗后视觉模拟评分法(visual analogue scale,VAS)评分均低于治疗前,治疗后观察组更低,差异均有统计学意义(P<0.05);治疗后,两组腹肌耐力、背肌耐力均明显提升,观察组均明显更高(P<0.05);两组治疗后功能障碍指数(ODI)评分均低于治疗前,观察组评分更低,差异均有统计学意义(P<0.05);两组总有效率比较:观察组(97.50%)高于对照组(80.00%),差异有统计学意义(P<0.05)。结论:在腰肌劳损患者治疗中采取中医推拿整复手法结合聚焦式冲击波治疗,可有效改善患者腰部功能障碍、缓解腰部疼痛,提升腹背肌耐力及临床治疗效果。Objective:To explore the application value of traditional Chinese medicine massage and reduction manipulation combined with focused shock wave therapy in the treatment of lumbar muscle strain.Method:A total of 80 patients with lumbar muscle strain admitted to Sanming Integrated Traditional Chinese and Western Medicine Hospital Affiliated to Fujian University of Traditional Chinese Medicine from December 2023 to May 2024 were included.According to the random number table method,there were 40 cases in each group of control group and observation group.The control group was treated with focused shockwave therapy,and the observation group was treated with focused shockwave therapy combined with traditional Chinese medicine massage and reduction manipulation.The clinical efficacy,pain improvement,functional impairment improvement and abdominal and back muscle endurance improvement of the two groups were compared.Result:The visual analogue scale(VAS)scores of the two groups after treatment were lower than those before treatment,and that of the observation group was lower after treatment,the differences were statistically significant(P<0.05);after treatment,the abdominal muscle endurance and back muscle endurance of the two groups were significantly improved,and those of the observation group were significantly higher(P<0.05);the ODI scores of the two groups after treatment were lower than those before treatment,and the score of the observation group was lower,the differences were statistically significant(P<0.05);the total effective rate of the observation group(97.50%)was higher than that of the control group(80.00%),the difference was statistically significant(P<0.05).Conclusion:In the treatment of patients with lumbar muscle strain,traditional Chinese medicine massage and reduction manipulation combined with focused shockwave therapy can effectively improve patients'lumbar dysfunction,relieve lumbar pain,improve abdominal and back muscle endurance and clinical treatment effect.
