医院集团模式下宁夏三级医院住院患者下转现状分析及对策研究

摘　　要：目的探讨宁夏某医院集团内患者下转的现状。方法选择我院2021年1月-2023年12月住院,并使用我院信息中心自主研发的医院集团双向转诊信息系统管理的患者为研究对象。分析患者性别、年龄、科室、下转医院级别、所在地区及类别分布。结果我院临床科室通过医院集团双向转诊信息平台共下转患者1572人,其中男862人,女710人,平均56岁。内科下转患者中男性占比66.63%,60~79岁年龄段占比48.47%;外科下转患者中女性占比61.55%,35~60岁年龄段占比50.61%。患者下转至医院集团内县级医疗机构的比例为47.71%,公立医疗机构比例为95.99%,综合类医疗机构比例为87.66%。结论我院临床科室向医院集团内医疗机构下转的患者主要以内科治疗、外科恢复期短、中年人群为主,应加强住院患者下转政策宣教,提高患者对下转的认知度,同时提升基层医疗卫生机构接诊能力,打通下转渠道,落实分级诊疗。Objective To investigate the current situation of patients with downward transfer in a hospital groups in Ningxia.Methods Patients who were hospitalized in our hospital from January 2021 to December 2023 and were managed by the two-way referral information system of the hospital group independently developed by the information center of our hospital were selected as the research objects.The distribution of patients'gender,age,department,transferred hospital level,region and category were analyzed.Results A total of 1572 patients were transferred from clinical departments through the two-way referral information platform of hospital group,including 862 males and 710 females,with an average age of 56 years.Among the medical patients,66.63% were male and 48.47% were 60-79 years old.Among the patients with internal medicine downward transfered,66.63% were male and 48.47% were 60-79 years old.Among the patients with surgical downward transfered,61.55% were females and 50.61% were 35-60 years old.The proportion of patients downward transferred to county-level medical institutions within the hospital group was 47.71%,the proportion of public medical institutions was 95.99%,and the proportion of comprehensive medical institutions was 87.66%.Conclusion The patients downward transfer from pilot clinical departments of our hospital to medical institutions within the hospital group are mainly medical treatment,surgical convalession period is short,middle-aged people,should strengthen the policy education of inpatients to downward transfer,improve the awareness of patients to downward transfer,at the same time improve the reception ability of primary medical and health institutions,open up the channel of downward transfer,implement hierarchical diagnosis and treatment.
