血脂、血同型半胱氨酸、血尿酸水平和帕金森病病情严重程度的相关性研究

摘  要:目的探讨血脂血尿酸(UA)、血同型半胱氨酸(HCY)水平与帕金森病(PD)病情严重程度的相关性。方法选取2021年1月-2024年8月就诊于我院神经内科的92例PD患者,设为PD组;选取体检中心92名同期健康体检者,设为健康对照(HC)组,检测两组总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白(LDL-C)、HCY、UA水平,并计算出残余胆固醇(RC),分析PD病情严重程度的影响因素,采用受试者工作特征曲线下面积评估血脂、HCY、UA对PD发病的预测价值。结果PD组TC、TG、LDL-C、RC、UA水平均低于HC组,HCY水平高于HC组(P<0.05)。二元Logistic回归分析结果显示,TC、LDL-C、RC、HCY、UA水平与PD发病相关(P<0.05)。相关性分析显示,PD组RC与病情严重程度明显相关(P<0.05),RC、TC与分期明显相关。结论血TC、LDL-C、RC、HCY水平与PD发病相关,TC、LDL-C、RC水平是PD的保护因素,HCY是PD的危险因素。RC水平与PD严重程度呈负相关,RC、TC水平与PD分期呈负相关。Objective To investigate the correlation between the levels of blood lipid,uric acid(UA)and homocysteine(Hcy)and the severity of Parkinson′s disease(PD).Methods A total of 92 PD patients who were admitted to the Department of Neurology of our hospital from January 2021 to August 2024 were selected as PD group.A total of 92 healthy subjects in the physical examination center during the same period were selected as the healthy control(HC)group.The levels of total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein(LDL-C),HCY and UA in the two groups were detected,and the residual cholesterol(RC)was calculated.The influencing factors of the severity of PD were analyzed.The area under the receiver operating characteristic curve was used to evaluate the predictive value of blood lipids,Hcy and UA on the incidence of PD.Results The levels of TC,TG,LDL-C,RC and UA in PD group were lower than those in HC group,and the level of HCY was higher than that in HC group(P<0.05).The results of binary Logistic regression analysis showed that the levels of TC,LDL-C,RC,HCY and UA were correlated with the incidence of PD(P<0.05).Correlation analysis showed that RC in PD group was significantly correlated with the severity of the disease(P<0.05),and RC and TC were significantly correlated with stage.Conclusion The levels of TC,LDL-C,RC and HCY are related to the pathogenesis of PD.The levels of TC,LDL-C and RC are protective factors of PD,and HCY is a risk factor of PD.The level of RC was negatively correlated with the severity of PD,and the levels of RC and TC were negatively correlated with PD stage.
