摘 要:目的探讨支气管哮喘患者血清膜型基质金属蛋白酶-1(MT1-MMP,又名MMP-14)、热休克蛋白27(HSP27)水平与急性发作期肺功能、气道炎症及复发的关系。方法选取2018年2月至2023年2月我院收治的140例支气管哮喘患者,86例急性发作期患者纳入急性组,54例缓解期患者纳入缓解组。采用酶联免疫吸附法(ELISA)检测患者血清MMP-14、HSP27水平,对比两组一般资料[年龄、性别、体质量指数、吸烟史、哮喘控制测试(ACT)评分],血清MMP-14、HSP27水平,肺功能[第1秒用力呼气容积(FEV_(1))、一秒率预计值(FEV_(1)%)、6 min步行距离(6MWD)],气道炎症指标[白介素-6(IL-6)、人免疫球蛋白E(IgE)、白介素-17(IL-17)、人转化生长因子-β(TGF-β)];分析血清MMP-14、HSP27水平与急性发作期肺功能、气道炎症的关系。治疗结束后对86例急性发作期患者随访1年,根据是否复发分为复发组和未复发组,对比两组血清MMP-14、HSP27水平。结果急性组ACT评分、FEV_(1)、FEV_(1)%、6MWD低于慢性组,血清MMP-14、HSP27、IL-6、IgE、IL-17、TGF-β水平高于慢性组(P<0.05)。血清MMP-14、HSP27水平与急性发作期FEV_(1)、FEV_(1)%、6MWD呈负相关,与IL-6、IgE、IL-17、TGF-β呈正相关(P<0.05);随访1年共有35例急性发作期患者复发,复发组血清MMP-14、HSP27水平高于未复发组(P<0.05)。结论支气管哮喘急性发作期患者血清MMP-14、HSP27水平升高,二者与急性发作期患者肺功能、气道炎症、复发有关。Objective To explore the relationship between the levels of serum MMP-14 and heat shock protein 27(HSP27)and the lung function,airway inflammation and recurrence during acute attacks of bronchial asthma.Methods One hundred and forty patients with bronchial asthma from February 2018 to February 2023 were selected.Among the patients,86 patients at the acute attack stage were included in an acute group,and 54 patients at the remission stage were included in a remission group.Serum MMP-14 and HSP27 levels were detected by ELISA.General data such as age,gender,body mass index,smoking history and asthma control test(ACT)score,serum MMP-14 and HSP27 levels,lung function such as FEV_(1),FEV_(1)%and 6MWD and airway inflammation indicators such as interleukin-6(IL-6),human immunoglobulin E(IgE),interleukin-17(IL-17)and human transforming growth factor-β(TGF-β)were compared between the two groups.The relationship between serum MMP-14 and HSP27 levels and lung function and airway inflammation during acute attack was analyzed.After the end of treatment,86 patients with acute attack were followed up for 1 year.They were further divided into a relapse group and a non-recurrence group according to whether or not they recurred.Serum levels of MMP-14 and HSP27 were compared between the two groups.Results The ACT score,FEV_(1),FEV_(1)%and 6MWD in the acute group was lower than those in the remission group(P<0.05).Serum levels of MMP-14,HSP27,IL-6,IgE,IL-17 and TGF-βin the acute group were higher than those in the remission group(P<0.05).Serum MMP-14 and HSP27 levels were negatively correlated with FEV_(1),FEV_(1)%and 6MWD,and positively correlated with IL-6,IgE,IL-17 and TGF-β(P<0.05).A total of 35 patients in the acute group recurred after 1 year of follow-up.The serum MMP-14 and HSP27 levels in the relapse group were higher than those in the non-relapse group(P<0.05).Conclusions Serum levels of MMP-14 and HSP27 are highly expressed in patients with acute attacks of bronchial asthma.The expression levels are related to lung fu
