改良强制性运动疗法联合针刺在痉挛型脑瘫患儿中的应用效果

摘　　要：目的:探讨改良强制性运动疗法(mCIMT)联合针刺在痉挛型脑瘫(SCP)患儿中的应用效果。方法:选择2023年6月—2024年6月牡丹江医科大学附属红旗医院收治的90例SCP患儿,按照随机数字表法分为联合组(n=45)和mCIMT组(n=45)。mCIMT组接受mCIMT,联合组在mCIMT组基础上接受针刺治疗。比较两组治疗前及治疗3个月后痉挛程度[改良Ashworth量表(MAS)]、粗大运动能力[粗大运动功能评估(GMFM)]、精细运动能力[精细运动功能评估(FMFM)]、平衡能力及步行能力[儿童平衡量表(PBS)、6分钟步行试验(6MWT)]、生活质量[儿童生存质量测定量表(PedsQL)]。结果:治疗后,联合组MAS评分显著低于mCIMT组(P<0.05);治疗后,联合组GMFM、FMFM各维度评分均显著高于mCIMT组(P<0.05);治疗后,联合组PBS评分显著高于mCIMT组,6MWT距离显著长于mCIMT组(P<0.05);治疗后,联合组PedsQL各维度评分均显著高于mCIMT组(P<0.05)。结论:mCIMT联合针刺能显著改善SCP患儿的痉挛程度、粗大运动能力、精细运动能力、步行能力及生活质量。Objective:To investigate the application effect of modified constraint-induced movement therapy(mCIMT)combined with acupuncture in children with spasticity cerebral palsy(SCP).Method:A total of 90 children with SCP admitted to Hongqi Hospital Affiliated to Mudanjiang Medical University from June 2023 to June 2024 were selected and divided into the combined group(n=45)and mCIMT group(n=45).The mCIMT group received mCIMT,and the combined group received acupuncture treatment on the basis of the mCIMT group.The spasm degree[modified Ashworth scale(MAS)],gross motor ability[gross motor function measure(GMFM)],fine motor ability[fine motor function measure(FMFM)],balance ability and walking ability[pediatric balance scale(PBS),6 min walking test(6MWT)]and quality of life[pediatric quality of life(PedsQL)]between two groups before and after 3 months of treatment were compared.Result:After treatment,MAS score in combined group was significantly lower than that in mCIMT group(P<0.05).After treatment,each dimension scores of GMFM and FMFM in combined group were significantly higher than those in mCIMT group(P<0.05).After treatment,PBS score in combined group was significantly higher than that in mCIMT group,and 6MWT distance was significantly longer than that in mCIMT group(P<0.05).After treatment,each dimension scores of PedsQL in combined group were significantly higher than those in mCIMT group(P<0.05).Conclusion:mCIMT combined with acupuncture can significantly improve spasm degree,gross motor ability,fine motor ability,walking ability and quality of life in children with SCP.
