雷公藤多苷与缬沙坦联合方案治疗慢性肾小球肾炎的效果及对肾功能的影响

摘　　要：目的:分析雷公藤多苷与缬沙坦联合治疗慢性肾小球肾炎的效果及对肾功能的影响。方法:选取2020年9月—2023年10月兖矿新里程总医院肾内科收治的100例慢性肾小球肾炎患者,按照随机数字表法分为对照组及研究组,各50例。对照组给予缬沙坦治疗,研究组给予雷公藤多苷与缬沙坦联合方案治疗,比较两组的临床疗效、症状缓解时间、细胞免疫功能、肾功能、炎症指标及不良反应发生率。结果:研究组总有效率高于对照组,蛋白尿转阴时间及血尿转阴时间均显著早于对照组,差异均有统计学意义(P<0.05)。治疗后,两组CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)、肾小球滤过率(GFR)水平均高于治疗前,CD8^(+)、血肌酐(Scr)、血尿素氮(BUN)、24 h尿蛋白定量(24 h pro)及尿白蛋白排泄率(UAER)水平均低于治疗前,且研究组CD3^(+)、CD4^(+)、CD8^(+)及CD4^(+)/CD8^(+)、GFR均高于对照组,CD8^(+)、Scr、BUN、24 h pro及UAER水平均低于对照组,差异均有统计学意义(P<0.05)。治疗后,两组白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)及超敏C反应蛋白(hs-CRP)均低于治疗前,且研究组均低于对照组(P<0.05)。结论:慢性肾小球肾炎临床治疗中雷公藤多苷与缬沙坦联合方案的应用可取得满意效果,有助于提高患者临床疗效,缩短症状缓解时间,并且还能促进其细胞免疫功能及肾功能改善,降低炎症因子水平。Objective:To analyze the effect of the combination of Tripterygium Wilfordii Glycosides and Valsartan in the treatment of chronic glomerulonephritis and its impact on renal function.Method:A total of 100 patients with chronic glomerulonephritis admitted to the Department of Nephrology,Yankuang New Journey General Hospital from September 2020 to October 2023 were selected and divided into control group and study group according to random number table method,with 50 cases in each group.The control group was treated with Valsartan,and the study group was treated with Tripterygium Wilfordii Glycosides combined with Valsartan.The clinical efficacy,symptom remission time,cellular immune function,renal function,inflammatory indexes and incidence of adverse reactions were compared between the two groups.Result:The total effective rate of the study group was higher than that of the control group,the time of proteinuria to negative and the time of hematuria to negative were significantly earlier than those of the control group,the differences were statistically significant(P<0.05).After treatment,the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)and glomerular filtration rate(GFR)in both groups were higher than those before treatment,and the levels of CD8^(+),blood creatinine(Scr),blood urea nitrogen(BUN),24 h urinary protein quantification(24 h pro)and urinary albumin excretion rate(UAER)were lower than those before treatment;in addition,the levels of CD3^(+),CD4^(+),CD8^(+),CD4^(+)/CD8^(+)and GFR in the study group were higher than those in the control group,and the levels of CD8^(+),Scr,BUN,24 h pro and UAER were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and hypersensitive C reactive protein(hs-CRP)in both groups were lower than those before treatment,and those in the study group were lower than those in the control group(P<0.05).Conclusion:The combination of Tripterygium Wilfordii Glycosides and Valsartan i
